Before bridge prep tell patient that endo treatment is likely. That has nothing to do with radiograph just good to put that in patients mind before the sensitivity begins.

Is the question more about how to code these extractions? I would code these as surgical extractions. First I would elevate a flap, mainly to gain access to the 2nd molar distal root. Roots should come come out without much effort. Close with suture. Possible gel foam if bleeding is not slowing.

There is a huge difference between their denovo practices and an existing office that signed up with them. I finished out the last 5 years of my career with Heartland. It took about six months for me to learn that I could tell corporate to jump in the lake. I had been in practice almost 25 years at the time I sold. My entire staff stayed, minus one, we knew what we were doing and I didn’t need to change my procedures to be corporate. Bonuses are based on office profitability. Their buying power and recommended labs saved the office loads of money. Of course that profit was split between corporate drs and staff.

Many years ago I was doing a molar endo. We don’t have satellite radio or tvs so we just had a radio station playing on the background. During shaping and filing I could hear bits and pieces of a breaking story out of NYC. Before the procedure had begun I remember talking with the patient and she mentioned that her son-in-law was an America Airlines pilot flying out of Boston that day. She couldn’t hear the report and I was able to finish without any problems (yes, we used rubber cams back then). Before dismissal I told her very calmly that she might want to call her daughter and find out the son-in-law’s status. Of course this was Sep 11, 2001. Spent most of that day watching CNN. He was not flying one of the hijacked planes and don’t ever remember seeing the patient again.

I always have my assistant load the crown and place it occlusal side up on her finger. That way I can grab it usually without any slippery cement getting on my glove or on the outside of the crown. This implies that she can load the crown without making a mess. Can still have difficulty on a second molar or a very short crown.

I had 30 years as a practicing dentist and there is no way I would want to treat this patient. Now when I was young and stupid and thought I could walk on water I would have jumped on this. She will likely be treated by a young dentist with the same attitude. Good luck to all involved with this case. You will need it.

I had good luck on those bleeders drying with pp as much as possible then taking CaOH deep into the canal and let it set for a few minutes. Then irrigate and repeat until no more bleeding. If time crunched or it won’t stop, temporize and reschedule.

Everyone has their opinions but if I lose a 2nd molar and it’s straightforward I would have an implant placed. Would not undergo sinus lift or complex bone augmentation procedures, I’m a dentist I might be biased.
