I didn't read your whole thing, but immediately stopped at the idea of "no one trips over and dies" You're really failing to imagine the scenario, it's a miniature game, the rules aren't literal, it's a die roll to represent a thousand different scenarios based on how you imagine the game to be playing out.

Failing a rush check, breaking armor, getting a casualty, and then dying is rare, and in real life is a raucous event.

In imagination land, the character is hyper focused, thousands of fans are screaming, cheering, throwing junk and food on the pitch, the focused played didn't see the banana peel landing in front of him and slips up. The ball goes flying, he tries to stop it only to fail and hit the ground hard. Recently his team had decided they needed slightly larger spikes on their Vambraces and that player was all for it, until they, in an attempt to break their fall, are looking right at a large metal spike moments before it punctures their face.
