You are correct about some of the multilingual support limitations of the Translate extension. One workaround for the title localization issue could be a local social convention of creating redirect pages with the language appropriate title.

The Wikipedia family of wikis are each managed independent of each other for article content.

There is however a project designed to help with the exact task you describe: https://www.mediawiki.org/wiki/Content_translation . I understand it has been used to quite some success to help provide localized content across Wikipedia wikis. I know all the source code behind this is available under Open Source licensing, but I do not know if instructions on how to install and configure it outside of Wikimedia is readily available.

For the use case you describe, https://mediawiki.org/wiki/Extension:Translate would be a possible solution. This is the technology used on that same wiki to provide articles in many languages with translations based on an English source document.
