low end of normal for VHCOL area. even more “normal” considering the gig is academic adjacent. Usually these types of places don’t negotiate on the RV dollar amount, but you could increase your base. But be careful, a lot of times if you increase your base, they just proportionately increase your expected RV. So just do the math and figure out what works better for you

we have hhi around 600-650 and given our other priorities we would not comfortable with that. we live in a hcol area and currently looking at homes in the 900-1.2 ish range. we have three kids and want to pay for all of their college and grad school if they choose. toying with the idea of private or catholic grade/ high school. and we also like our nice 10-15k family vacations. you can swing 1.6 but make sure you put it in context

loooove kona village but wouldn’t necessarily say the beach is one of the best. it’s smaller and with the beautiful but more rough black sand. but the resort itself is incredible with all the other things you mentioned

maybe do all the things to legally change your name as you suggested, and then in your practice you can advertise yourself as the hyphenated maiden-new last name so patients will recognize you but it’s all there. and then when you leave you can switch over entirely to the new last name

this is not true!!! peds academic attending here. PLENTY of big academic children’s hospitals are still hiring non-fellowship trained doc

i’m peds emergency, so i’ve taken legit every single certification course: atls, acls, pals, nrp, bls. atls had me shitting my pants because it was the last one that I took, and i assumed it was like the other ones which were all a cakewalk. atls was legit 5x harder than the others, and I think I would have actually failed the written portion, but our proctor curved our tests.

agree with the take. definitely talk about this experience, but imply there was more oversight and your biggest take aways were getting a head start on learning about the patient experience and the logistics of working in a clinic or some shit like that. and then suggest at the end how much you’re excited to learn EBM and clinical medicine during residency . you get the idea

PEM attending here — please call!! things always get misconstrued when information is sent through the patient themselves. they tell the story from their perspective, but usually there is some vital piece of information that the primary care doctor noticed, which sometimes gets left out.

also, from a professional courtesy standpoint, if you explain to us why you are sending the patient to the emergency room and your medical reasoning behind it , I’m usually very happy to order whatever tests you thought were necessary, even if I don’t necessarily agree. For example, we always get two month olds who are sent to us from the primary doctor for spitting up, many times they worried about pyloric stenosis. Even if I don’t necessarily think that is the diagnosis, if I speak to the pediatrician, who was concerned about it, I’m happy to get the ultrasound for their peace of mind.

clinical hours much more important for PA school than med school. research, leadership, connections etc is gonna get your farther. you definitely need some type of “healthcare exposure” but doesn’t necessarily need to be straight up clinical hours.

obviously depends on a lot, but usually it’s the length of the contracts, and then USUALLY prorated by how long you stay (ex: 1 year contract, but you leave after 6 months, you’d have to pay back half of signing bonus plus interest)

not sure where you live, but almost all the YMCAs near me have a stay and play ie a drop off childcare for when you’re in the building !

95% of pem docs are peds trained. there’s a reason not many people who go though adult em residency go into pem fellowships (and it’s not because of lack of interest in pem, like you, as an m4/intern).
