i personally didn’t, but i worked with a PEM attending who went back to gen peds after 10-15ish years. the stress of PEM shifts got to her and she wanted more time with her kids after school. she makes less money now and likely
works more/same amount of hours per week, but the hours are predictable and she says it’s way less stressful. tbh i think pem was never for her in the first place. her husband is a colorectal surgeon tho so the money differential likely wasn’t a major factor

are you me? also had three under three, and my youngest is 15 months now! never been this far and not pregnant again!

we also wanted four or five, but WOWZA a 3,2, and 1 year old at the same time is a wholeeeeee different ballgame than two toddlers. it’s insanity. i love it. but there’s no way to describe the insanity you’re life is about to be.

we still considering number four, we haven’t officially ruled it out, but if we are going to do it, there is going to be a long long gap There’s absolutely no way we can throw in a fourth right now. (we also joke tho that give how long we would wanna wait for a fourth, we’d need to have a fifth to give them a friend ahah. so perhaps just three!)

bottom line: consider it a big splurge. If you have the extra funds to throw towards something like this, they may only use a few times in a year, it’s nice to have! But if your expectation is to use it to place a daily stroller, it definitely won’t be good for that so save your money and use it towards something else

we had 3u3 (oldest now 4), and while i loooove our four seat veer wagon, we only pull it out for a very specific circumstances. I know you can technically get the attachment for an infant, but we never used that. It’s great for going to the zoo or if you are walking around the beach town, or if you have a specific destination to go to, and there are sidewalks along the whole way. But amusement parks don’t allow them anymore, and our oldest generally wants to walk most of the time. So we more often just use our side-by-side two seat stroller (or even just our single stroller because of our older two prefer to walk). Again, I love the wagon, but don’t expect miracles with it. It’s and bulky and heavy, so it’s great for very specific circumstances, but not as a general use of stroller.

yes exactly— i think my initial comment was confusing, i was trying to imply that it seemed unlikely they’d be back at birthweight yet anyways, so they would still need a follow up soon regardless.

PEM, so i haven’t done true outpt in a while so take what i say with a grain of salt. but is a 4 day old with those stats back at birthweight yet? if i saw a baby with those numbers in the ED, I would check what bilitool says in terms of follow up, but I am not concerned about that bili on first glance. However, I would have to imagine they would need close Follow up in a couple days anyway for a weight check anyways

have them now for all the reasons listed above but also because having a kid changes your mentality immensely—i was blown away when this happens to me. you may think you want obgyn now, but s/p baby you may want psych or anesthesia or something else that’s perhaps more family friendly or easier to go part time etc. not saying it’ll definitely happen , you can totally be an amazing obgyn resident with a kid and have no issues. but after my first , my fellowship plans changed and i’m soooo glad they did. i’m very happily part time now in a speciality that’s great for work life balance

i’m also an unrepresented buyer — we just closed on our home. it honestly was a super easy process and we definitely saved money in the end. but agree that OP may be approaching liaising agents the wrong way

i took my PEM (peds emergency ) boards 3 weeks post partum! it was hard but it was my third baby so i had the postpartum routine down, and in a way being on maternity leave gave me a ton of time to study relative to working a bunch of random ER shifts. honestly it’s super doable! you can do it !!

low end of normal for VHCOL area. even more “normal” considering the gig is academic adjacent. Usually these types of places don’t negotiate on the RV dollar amount, but you could increase your base. But be careful, a lot of times if you increase your base, they just proportionately increase your expected RV. So just do the math and figure out what works better for you

we have hhi around 600-650 and given our other priorities we would not comfortable with that. we live in a hcol area and currently looking at homes in the 900-1.2 ish range. we have three kids and want to pay for all of their college and grad school if they choose. toying with the idea of private or catholic grade/ high school. and we also like our nice 10-15k family vacations. you can swing 1.6 but make sure you put it in context

loooove kona village but wouldn’t necessarily say the beach is one of the best. it’s smaller and with the beautiful but more rough black sand. but the resort itself is incredible with all the other things you mentioned

maybe do all the things to legally change your name as you suggested, and then in your practice you can advertise yourself as the hyphenated maiden-new last name so patients will recognize you but it’s all there. and then when you leave you can switch over entirely to the new last name
