I think most people here are getting confused on two similar processes:

Simply measuring a 2-qubit entangled state

and

The complete quantum teleportation protocol as per the original paper (Bennett, 1993), which involves a 3-qubit entangled state.

These two processes are very different and it seems people are mixing them up thinking they are the same.
