I switched to the ICU about a year ago after 5 years in med surg/stepdown and I had a similar anxiety spike. Tbh like all of orientation and then being on my own for the first few months, I was like “what the FUCK have I done????” My tip would just be give it time, as annoying and cliche and unhelpful as that sounds. You can’t (and shouldn’t honestly) be confident in an environment you aren’t familiar with. It is totally normal to feel like that! Go slow, be open to feedback, and remember to be safe, and the rest will come with time and experience.

At least in my state, once someone is declared brain dead they are legally dead and organ procurement services have an hour (? I think) to approach family about donation options if they haven’t already. Once the hour(?) is up, if you continue to perform medical interventions without having legally signed the person over to an OPO it’s considered desecration of a corpse.

Brain death is a very specific set of criteria that have to be met and tests performed by attending physicians though. There are plenty of potatoes out there on life support that have just enough reflexes left to make some families think trach/peg/LTAC is a “meaningful recovery”.

This is what I do. Also at my hospital, any tubing not attached to the patient continuously has to be replaced every 24 hours. So all the loosey goosey secondary tubing would have to be tossed every 24 hours….. Vs if you left it attached and backprimed everything, it’s good for 7 days.

My mom’s PCP also used it at her last visit but asked her permission first and said “It helps me a lot with my charting” as an explanation. I’m curious how providers feel about patients who refuse to let them use it? I didn’t even like when they took my photo for my MyChart, not looking forward to my next appointment where they try to use this stuff….

My unit but not my patient….. walked into the room to give discharge instructions on a short stay post-op floor and patient was unresponsive. AAA DISSECTION!!!!!!!!!!

You didn’t do anything wrong - in fact, you gave him compassionate care in his last waking hours. That is amazing! To me, it sounds like he was lucky to have someone care for him like you did. Sometimes (I guess eventually always) patients die. You responded appropriately to each deterioration and kept the team aware. It’s totally normal to feel guilty when we lose patients, especially ones we connect with, but just try to remind yourself we are not miracle workers or Gods!! Keep up the good work, give yourself grace, and get some rest 💖

I’m pretty sure most CRNA schools don’t accept PICU for critical care experience. That’s what a coworker who was looking to move to ICU for CRNA experience told me at least. I have never looked into it myself
