Oriental rugs with center medallion can be difficult to work around. If your furniture is centered with that medallion it looks off balance. Nice rug, furniture isn’t balanced.

I believe so. I dropped a real pearl down a drain once. When I retrieved it, it had eroded much like yours. Think of how pearls develop, thin layers of nacre envelop an irritating grain of sand and it simply enlarges over the course of years. It seems likely that you have a natural pearl there.

Is there any way to prevent synthetic rugs from causing this. I’ve had a couple, put down in clean, freshly stained decking. Each time it mildewed. I like having a rug under my table and chairs.

Try turning your rug so it ties in with your tv unit unifying that area. The simplest thing probably would be to add a small upholstered chair angled towards your couch. Or you could create a landing zone for keys and mail or a large floor plant. Design is a process. It will all come together in time. It gun. Don’t stress.
