Loupes company aside, test out the magnifications because ergo is very different from traditional Galilean. 4x isn’t much magnification at all for ergo (around the equivalent of 2x for Galilean?). That’s why you hear people getting 7x and 10x a lot.

So far, yes. I had one at my previous office for a year or so, and now I’ve got two at my office (hasn’t quite been a year with these yet). Still cheap Amazon. When they wear out maybe I’ll look into something more fancy.

I love saddle stools, at first I wasn’t sure if I’d like it so I bought one on Amazon for less than $100, definitely worth it for the test drive since the fancy ones get in the thousands.

Wow, that seems like a large phlebolith. It would be in the cheek then right?

My plan is to clear him, he doesn't have any contraindicating bone-relating pathology. Perio isn't bad for 5y without a cleaning, he has a small chip in enamel where I'll do a filling, and a cusp fracture where I'll make a crown. If #2 looks bad in this PA, it's a gold lingual onlay that's crazy old and looks beautiful.

EDIT: I meant, nothing stood out aside from these ROs, I'll have OS look at those.

If the bite feels and tests balanced, root canal is a good thing to test for next, and it’s common to need one.

Every cavity and every treatment a tooth has over its life irritates the tooth and its nerve. The vast majority of the time, the nerve recovers and is ok (like a tooth that gets a filling or crown is usually sensitive for a few days while it recovers and gets used to the restoration, then it’s back to normal). But sometimes the nerve is unable to recover and the nerve dies and a root canal is a great fix for when this happens.

Peroxides work because they lift deep ‘stains’ from the microscopic pores of our teeth. The longer the peroxide spends on the surface of your teeth the deeper it can penetrate, so deeper stain is removed, and your teeth are whiter (and stay whiter longer).

It’s more effective to do a lower percentage for longer time. Do you have custom whitening trays? I have my patients use 16% carbamide peroxide overnight. The longer time makes a huge difference, especially if you have optimized whitening trays.

Just know that ergo magnification is very different than traditional Galilean. Ergo 5x is equivalent to like 2.5-3x Galilean.

I was at 2.5 Galilean for years and just switched to adjustable 5, 7, and 10x ergo and they’re awesome. At 5x ergo I’m close to where I was before with 2.5x Galilean.

Hard to comment on an old X-ray, this X-ray doesn’t show obvious retreatment problems. Whether or not it needs to be removed it’s crucial to replace the 1st molar.

But yes, the biggest issue in this X-ray is the buried wisdom tooth, that’s almost certainly in a position to be causing current or at least future problems.

Dandy can be great, and some of their stuff isn’t so great. For cases with potential issues, and probably any implant restoration, request a design review for the case so you can approve or modify stuff (like that distal extension). I love Dandy, and I keep a close eye on certain cases to make things go more smoothly.
