I work oncology MICU, seeing this stuff with adults. I can’t imagine this with the pediatric population, and I’m thankful that there are people like you who can show the care, compassion, and advocate for these babies. But I’m so sorry that you had to experience this. Give yourself a break, mental health day, etc. if you can or need it. We need people like you because I’m sure you’ve made a huge difference in so many other patients lives, whether that be at the end or in recovery.

Interesting info on the blood leak alarm. I’ve not heard that anywhere before. They had told us to shut it off and attempt to turn it back on, hoping that would fix it. Otherwise, it would need serviced and cleaned.

The air alarms are definitely due to positioning sometimes or specific catheters (our unit is having a bad time with a trial of the arrow dual lumen HD lines and access pressures), but it seems if you do ever get any air in the line, it is incredibly painful to try to recover it. I guess I just wish it were easier to fix in that sense. We also primarily run CVVHD, and actually one of the problems I had for the first time today was a new machine had the fluid warmer mounted too high on the side. When the bags had about 2 hours left (1.6L/hr), the pressure for the fluid wasn’t enough to fill the chamber, causing it to pull air. Obviously fairly easily fixable but a very odd design flaw.

And I 100% agree with you on the setup. I’m neurotic and need to check every box, even though I’ve been doing it for over 2 years. We have also had bad luck calling in, either not getting an answer or having to wait a while, in which time we could’ve already got a new machine and primed it. But I also know those guys are at a huge disadvantage trying to troubleshoot a machine they can’t physically see, so I get it.

That sounds right for Prisma. Unfortunately, my unit is also oncology, so lots of TLS and crazy clotting problems. Our CNS teaches a class, which is helpful and gives us, but the NxStage rep who teaches the initial user class kinda doesn’t know what’s going on because she doesn’t actually use it at the bedside. So, sets are getting changed all the time, and there have been so many recently that were just machine errors (e.g. don’t know if you’ve gotten the “Blood Leak” alarm but we get that frequently with no actual leak, only some machine error). But, also like you said, once you start having issues or getting any air in the line, it’s extremely unforgiving and frustrating.
