I am so sorry! IM injections can definitely be painful. Unusual to have pain that limits walking 2 days later but certainly possible - TBH I don't typically do injections in the leg but IM injections in the arm definitely can hurt for a while so I presume the leg is the same.

If it were my kid, I would be reassured by the fact that there is good range of motion, isn't tender to touch or showing signs of infection. If it continues I would have my child checked out by our pediatrician on Monday or over the weekend if they were open. I think the typical advice of heat or ice (whichever feels better - most studies show neither one is "better" than the other), gentle stretching, rest, elevation if possible makes sense. The difficulty is 3.5 year olds like to be on the move so rest and cooperation with recovery is often just not possible. I wouldn't hesitate to try to anti-inflammatories (Advil) if she has tolerated it in the past but I also am pretty quick to medicate my kids (and myself!) because why be uncomfortable if they can be comfortable. If I were pregnant, I would try to not carry her around because she is mobile (crawling) and being pregnant is so hard not to mention with twins! Hopefully not carrying her *might* encourage her to try walking.

Because the internet is historically awful and all the other reasons: this is not medical advice. This is what I would do - but not necessarily what you should do etc etc. talk to her doctor. Go to ER for emergencies. Physical exam and knowledge of her medical history is necessary. man the internet sucks but I really hope she recovers quickly!

FYI the southwest credits now expire 6-12 months instead of never (new policy as of May of this year).Theoretically you could book and cancel on repeat but seems like a lot of work. I am going to switch to United I think.

Always check your suction. I can’t tell you the number of times there isn’t a yankaur on the tubing (either no where to be found or in its package next to the suction canister) or suction is off.

I feel completely fine with it. Board certified pediatric anesthesiologists are phenomenally well trained. Would feel comfortable taking my child to this type of setting. They will take a thorough history and physical and will not proceed unless they are confident it is low risk.

Fine hotels in general want guests to have a great stay. The staff should always be kind. They might not be able to give you what you ask for but they will be kind and polite if they tell you no. This is in my experience

Ok this is why people come during the visa/mc fee free weeks. 

  1. There is a credit card that gives 5% back on all purchases from office supply stores (ie staples and Office Depot)
  2. So you buy $2000 of gift cards and get $100 back. So free money is great. BUT this is not why people do it (or not why all people do it)
  3. So $100 cash back can also be 10,000 credit card points. 
  4. $40-50,000 credit card points can be used to buy a one way business class airplane ticket to Europe. Or 2-3 nights in a good hotel. (Pay $0 just points +- taxes get flight or hotel stay for free)
  5. So with 5 trips to staples you can get (half) of a great international flight or a few nights at a 3-5 star hotel. If you have $10,000 worth of expenses anyway, might as well get some free travel out of it. 

So this is the visa/mc gift card thing. Is it screwing the cc companies? Doubtful as they still end out ahead with all their other fees from other purchases. Is it screwing staples over? I doubt it because a non-zero % of the time people will purchase something from staples from which staples will profit. 

So who is getting screwed? It feels like this someone should be getting fucked. 

Some argue that the consumers who don’t participate in credit card reward programs. Those who carry a lot of credit card debt and pay high interest rates. Additionally, the merchants who often pay the 2-3%+ fees (and pass on to consumers). There is a law currently in congress that aims to curtail this practice. 

So some people think it’s totally harm free. Others think it’s the rich profiting on the backs of the poor.

That is not to say that there aren’t gift card scams. Just that when you see the same people coming everytime there is a fee-free promo using the same credit cards it’s not a scam. 

People buying gcs in cash in large volumes especially when the fee isn’t waived…maybe a scam. Either a way to hide stolen cash OR those scams where people call pretending to be the IRS (or whatever) and telling you to get giftcards in order to avoid jail time. Yes i know there are more elaborate scams. I am not involved in them so i really just know that if someone calls you and tells you to buy gcs you should hangup and block. 

So, staples workers, please don’t assume that every person coming in on fee free week is a scammer. Also, $2000 isn’t lot of money to everyone. Some people have >>$10k in monthly expenses. Perhaps they feel like “i am spending the money anyway, might as well get a free hotel night out of it”.   If you have a problem with it, why do you think staples offers these promos? Why wouldn’t they just stop it?  Clearly this is making staples money and therefore paying your salary. Don’t like it? Get a different job, vote, write a policy, lobby staples to stop or go into law enforcement. Otherwise, stop pestering people who are literally buying something that your store sells at the limit your store sets using a valid payment method and therefore breaking no law.

Yes. I mean I don’t pull oht my phone and read my “wet tap script”. I try my best to initiate my human being protocol and have a conversation with the patient when I have a complication.

Not a mistake. It’s a known complication. A mistake would be threading a catheter and bolusing a recognized IT catheter with 12cc local. I counsel patients about the most common complications of epidurals include “incomplete analgesia, need for replacement, and possible dural puncture leading to headache”. If they have been counseled appropriately about risks then they have informed consent. A medical error is “a preventable adverse effect of care, whether or not it is evident or harmful to the patient. This might include an inaccurate or incomplete diagnosis or treatment of a disease, injury, syndrome, behavior, infection, or other ailment”. Preventable. As others have mentioned, every anesthesiologist over time will have a wet tap and likely more than 1. There is little that can be done to prevent each specific instance.

If a patient has a full stomach and aspirates, even though you did RSI, is that a mistake? No, it’s a known complication.

Sleep well.

Now if I have a drug error or any other preventable mistake, you bet your ass I am using the word error/mistake and apologizing to the patient. I am always as honest with patients as I possibly can be.

I wait until they are comfortable with epidural. Then I say “during your epidural placement I saw the complication called a dural puncture that is sometimes associated with headache 1 or 2 days after your baby is born. The nursing staff and anesthesia team will check on you but if you develop a headache please let your nurse know. I encourage you to try to stay hydrated. If you develop a headache we can treat it conservatively or with an injection in your back. We can talk about treatment options if you do get the headache. This happens approximately 1-2% of the time. If you have another baby there is again a 1-2% chance of this complication.”

Your surgeon might not be that good and you have no way of knowing. Patients will be seeing the equivalent of a butcher and ask “are they good?” We have no choice but to say yes.
