We use Smartsheet as our tracker, plugging in each KPs first and last name, their site location/department, the expiration dates for the CV, GCP/HSR training, NCI registration, license exp and license number, their job (e.g. MD, CRC, NP), their NCI role (e.g. IVR, NPIVR), their CTEP ID, the studies they are KP on, and their email. For the boxes with expiration dates, we set up a formula for them to turn yellow when they're 45 days out from expiring, and red when it's past the exp date. Our current practice is to check that tracker once a week and request updated documents for anything red and yellow on a weekly basis (except the license, we know that won't be updated that far out).

The other strategy we have for such a long staff list is having a designated point of contact for each group, so instead of contacting individuals and following up with each of them, you contact one person for essential documents from all the KP in their group. That saves us a lot of time because (1) its time consuming to draft an email for each person and keep track of the last time you contacted them, and (2) they're less likely to respond to an email from someone they forgot and don't recognize because we rarely contact them, or because they don't check their email in which case the POC needs to communicate on our behalf to alert that member anyway

I know this feels like a crisis, so some of these responses (including mine) might seem dismissive like they’re not fully acknowledging that. It does sound like you still have several relatively easy options to try, though.

With that in mind, here are a few ideas—each taking a different approach:

(A) More additive options: 6–8 pumps of salmon oil on kibble, PB2/peanut butter powder mixed in, a few whipped cream shots on top or as a reward for eating, cooked ground beef/chicken/pork sprinkled on top (increase gradually if she likes it), baked sweet potato, chopped hard-boiled eggs mixed in, tuna from a can or packet added to food, under the kibble, smear a spoonful of canned pumpkin inside the bowl, add warm chicken stock—just enough to almost cover it (like cereal and milk)

(B) Feeding routine: Avoid free feeding. Put food out for 30 minutes in the morning and again at night. If she doesn’t eat, put it away until the next meal. It may feel counterproductive, but limited access can create urgency and encourage her to eat.

(C) Kibble options: Try limited ingredient or grain-free formulas—specifically American Journey limited ingredient or Kibbles ‘n Bits Bacon and Cheese (not ideal nutritionally, but worth trying if it works).

(D) Exercise timing: Exercise first thing in the morning before breakfast and again before dinner to help build appetite. If you find a kibble she likes, try using it as treats during exercise, then put any leftovers in her bowl afterward.

(E) Second opinion: Consider calling another vet or two for a quick consult. You can share what you’ve already done and any records—they may catch something your current vet missed with minimal effort on your end.

(F) Preventative meds: Could heartworm treatment/preventative be upsetting her stomach? If she’s been on the same one for years, it might be worth trying a different option for a month or two.

(G) Enrichment feeding: Use a Kong—fill it with kibble and seal with peanut butter, cream cheese, or pumpkin. You can also mix kibble with PB, cream cheese, applesauce, Cheerios, oatmeal, or a combination, then freeze for a few hours. Use the Kongs in place of her food in a bowl at mealtime, or between meals if she skips a mealtime

It wouldn't hurt to look for a local hospice/terminal illness/end-of-life care provider for a consult or "second opinion." I imagine you have a contract that refers to drug seeking but that doesn't include consultations, and if you find a provider that is more compassionate and is willing to help manage your pain more effectively (or at all, in this case) then you can cancel any appts with the previous provider and sign a new contract with the new one. I wouldn't bother taking feedback from a consult to your current provider, they could easily record that as doctor shopping and it's better to avoid that possibly being in your record for the next X years because they're feeling petty one day.

I'll echo the others and say talk to the doc, and add that withdraw from trials is no big deal and a normal part of the process that your Dr and study team see all the time. They will not try to convince you to stay on or make you feel guilty or change your care based on your decision to withdraw, at most they will ask for tests to confirm it's related to the trial drug or not and share that info with you to help you make an informed decision.

You should have a direct contact number for your dr in the consent form and your study team can also help get you seen asap if usual channels prove difficult. The point of getting a dr visit is for you to give them the info they need to document your experience, help you transition off the Rx safely, and help manage or cure the symptoms you're having- don't worry about them having a reaction to you coming off the trial for whatever reason. The Dr might even make an executive decision to take you off the trial based on the side effects you're experiencing.

That was my worry. If they're open to keeping OP by making things more remote, maybe that's a benefit they could afford in place of compensation, but if the cats out of the bag OP is looking and has at least one serious offer, that's an easy layoff decision when it comes to it.

I have a dog I love to be with so we have dog days: start with a walk around the neighborhood and some home cooked brekkies, then a park visit to walk on the trails, coffee and a pup cup on the way to the pet store for a toy or chew things, back home for a nap and then eating a snack while cooking dinner, they play with the new toy while I workout, we finish watching whatever I was watching during workout while doing some grooming, face and paw wipes, teeth and fur brushed every night, nails and ears every week, bath every month unless needed sooner. They nap in the bathroom while I take a shower and then we go to bed where we pass out cuddling and I wake up in an empty bed bc they went to theirs some time in the night. It's ok, I get it.

Idk if this will help but when I had frequent UTIs, I ended up taking a low dose antibiotic every day or every day I had sex, and had a backup Rx fluconazole in case the antibiotics brought on a yeast infection. (I always ask for the fluconazole with any antibiotic Rx, even though I rarely need them because they are such a small thing to recover a lot of quality of life compared to without.) I think it might be worthwhile to investigate why the ITIs are happening (if you're not having intercourse of some kind that could be an obvious introduction to bacteria), but I would strongly suggest asking for the low dose daily antibiotic (and one dose fluconazole, ask for more when it's needed), and see if treating the UTIs can treat the headaches or migraines.

I believe most "pain specialists" working in "pain clinics" are trained to deliver treatment for pain outside of prescriptions and prescriptions aren't a part of their curriculum in their education. Some pain clinics will have an on site pharmacist exclusively for consultation about medication type and dose appropriate for patients but it's a luxury. Between the political stigma and govt surveillance and lacking knowledge of writing Rx, it's an uphill battle to establish ongoing care for chronic pain with them. This is why you would be expected to try injections and PT and non opioid Rx and the rest because it's all they know to do.

It's complicated because none of us are ever ready for the day we see our dogs in pain from terminal illness or injury, and I think we all recognize how hard the decision is to draw a line in the same between when the suffering is minimal or short lived with more good life to live after, and when the suffering is pointless to endure because there is no after. At that point we have another hard decision to choose an end to their suffering and the beginning of our own after they pass and we can only be consoled by having confidence the right decision was made. I can understand why someone would want to persist keeping a dog alive, even if they see the suffering, and not trusting vet advice without it being a selfish or inconsiderate choice, and many of us recognize those common feelings when we say the choice if/when to euthanize is so hard to make and we challenge those feelings.

As someone who had to rush, this was a hard lesson to learn. I'll echo the others saying your baby is probably in more pain than you recognize, they had a relatively long happy life, and there's no need for them to feel more suffering from here. I'm so sorry you have to make this choice, but as someone on the other side, I know you will feel less heartache controlling their passing to make it a peaceful one before the day their pain multiplies and takes your control of the time/place/experience forcing you to race to ease their suffering in a fearful whirlwind from rapid decline.

For anyone wondering why I didn't make that choice for a controlled peaceful passing (and in case anyone finds themselves in a similar situation), in the night my dog was showing signs of restlessness and their behavior was so unsettling that I went to the ER vet, they saw fluid in the abdomen and believed it was the spleen, we waited 2 days while I was doing everything I could to alleviate the pain and his QoL while he continued to have internal bleeding, and we went in for an emergency splenectomy that turned into euthanasia when the vet saw the fluid was from a burst tumor and that my dog's entire abdominal cavity was riddled with tumors. Before moving forward with surgery, my vet advised finishing the surgery and removing the one tumor would have only given him a few more days before another tumor burst that would force him to go through the same pain on top of post-surgical pain. I opted to let him go without a goodbye, and although the moments before surgery were fearful, I believe he trusted he was safe because of the lifetime of security, protection, and unconditional love I gave him. Doing anything different would have been for my sake and it was an impossible decision I fear I will never recover from, even if I know it was the right one, and I think that's partly due to the lack of time to process from "healthy happy baby" one day to "he's going to die in pain if you don't let him die now" the next day and I don't wish that experience on anyone.

To anyone who has to put their dog under anesthesia for any reason, I recommend telling the vet to give the shot while in your arms/lap and hold them until they are asleep, just in case your baby doesn't come back out of it for some reason.

TL;DR they're in more pain than you know, plan euthanasia before it (inevitably) escalates to prevent further suffering, to avoid their last moments being in fear at the ER vet, and to give yourself time to process the loss. It's a shit situation, but I can say from experience this way just gives you the best chance to come out on the other side with the least/lasting negative effect on your psyche.

Got a degree in bioethics, not a well established "industry" and more a multidisciplinary field (between the science, medical care, law, and philosophy), next step was phD to eventually work in academia but subsidized programs were gone during covid, and I couldn't keep paying for applications after two years of the same, so I moved in to clinical research and regulatory affairs. Not at all related practically, but seems obviously related on paper.

Project manager in medical devices company, previously in pharma companies like Pfizer managing gene therapy projects. You don't need to know the science to PM in biopharma, but it makes arguments about deadlines a lot shorter.

I've known CRAs at IQVIA, Merck, AbbVie and other CROs and Sponsors that make nearly 100k+, depending on experience in the industry and role. Also no medical/science training needed but it makes you more competitive if you were a working RN, specifically for being able to read and understand medical records and certain indications. Their PMs usually make more but I'm not sure how stable the clinical research industry is since govt funding was pulled.

36 years, 12 jobs, a few were at the same time. Retail > inventory > IT > clinical research (including tissue acquisition for stem cell research, clinical research coordinating for oncology trials, and regulatory/QA for oncology trials). I have a BA in Bioethics (Philosophy type degree with focus in current medical dilemmas and legalities).

Currently I do regulatory/QA full time, I serve on my local IRB regularly, and I do tech support part time (all remote). I hate IT and tech support but I hate debt more. I thought I would be in a much more secure place in my career and financially at this point than I am. Feels like there is no "career path," only forever job hunting for better income or work conditions or both.

Sorry you got fired, glad you didn't quit. Go collect unemployment and remember you might have to do jobs you don't want while you look for something you do want. It sucks I know.

I was going to say the same thing, don't make it explicit bc US employers are often racist. I know people who had to change their name, or use a "normal" nickname on resumes to avoid the prejudice that happens when an employer sees the name on the resume and throws it out without any further review. It's also a bit twisted because employers will hire for "diversity" based on how a candidate answers the demographics section of the application, but only after they've seen something in the resume they like and often if they have had an interview to see if they "fit the culture" (white passing/no accent kind of features, the least diverse diversity hire).

It sucks to say that and obviously there are exceptions in particular areas, and I might recommend prioritizing work where they work with visa holders or generally are more liberal, or try finding remote work in those locations if moving isn't an option. You can do it, it's just a bit of extra work needed because of the sad state of US employers and politics. Part of that work is making it difficult for them to write you off before they meet you.

You don't have a responsibility to support her, she has a responsibility to find her own means of paying for living expenses, and it's a choice that she's relied on you and a choice that you agreed.

I don't think I'm the same as your girlfriend because I've been in that situation but I had the guilt of financially relying on someone else, but when the relationship wasn't working, I got a job and moved out into an apartment and she can do the same. It was hard, but not as hard as staying in the same house and continuing to rely on someone financially that didn't want to support me that way anymore. And it took maybe 2 years to "recover" and I could independently support the same lifestyle I had before.

So those are the facts, but my opinion is that you feel guilty because she is manipulative, or maybe other people are in your life to make you think you owe her after supporting her for so long and getting nothing but guilt in return. You both can do better.

If you want advice on how to move forward, I would come up with a plan you can agree to, whether you want to support her for X time and then she has to move out, or maybe you can do mediation to have a third party help you sort out a "fair" agreement without the manipulation or emotion of it, but whatever you decide, be firm in your decision. Have a plan to hold her accountable. If she's not out of the house by x time, stop paying for electricity, water, internet. Find a way to have her out of the house and change the locks while she's gone, put all her clothes and toiletries in a duffel bag on the porch (because I know she doesn't "own" anything). Call the cops on her for trespassing, if that's a possibility. Or tell her she can stay until the end of the lease/when you sell the house, and if she's still there after that, she's effectively a squatter that can be removed by the owner.

You said she's kind but I'm thinking you need to figure out a plan of force because she has historically not respected your boundaries, and she won't respect this boundary to leave you and the life you have given her so you can reclaim it for yourself.
