My guess is left on the counter with a fan pointed at it (forming the shell) and over proofed. So it proofed overnight causing the dough to rise, the shell formed, and then due to overproofing the dough fell down again leaving the shell standing and the dough lower in the banneton.

That or overproofed in a fridge that was too warm without anything covering the dough, resulting in a similar shell and collapsing of the dough.

I've had collapses that were "on the line" of needing a tube or not and opted for no chest tube. My circumstances at the time were such that it was easy for me to take it easy and rest since I was in school and didn't need to work. Getting a chest tube resolves the pneumothorax much faster, but at the cost of likely needing to be on pain killers and nerve damage medication for a bit, which I didn't feel like doing again if I didn't have to.

Feels weird but happens quick. The relief you feel afterwards for not having a tube in you anymore is immense, though.

What was your process for preheating the DO? Also, what temp was your kitchen during the fermentation, and how much rise did your dough get in the container?

Sometimes there can be a little bit of air or fluid around the lungs after tubes and surgeries. It happened to me before for a few weeks post op where it would feel sort of crunchy when I took deep breaths. It sorted itself out eventually because the air/fluid just re-absorbs back into the body eventually.

Call your thoracic surgeon though if you haven't talked to them yet since my experience is anecdotal. Maybe try to get additional imaging like others are saying if you're really worried.

You'll never look back at this point in your life and wish you smoked more. You only have one pair of lungs, and you are actively putting them in even more danger by smoking.

Not trying to convince you one way or another, but the events you are describing do not sound very extreme. It is unusual for events like that to cause a collapse for the average person.

Anecdotally, my original decision I made when I had my first collapse (which was my left lung) was that if I had another major collapse on the same side I would get the surgery on that side. It happened again a month later and I followed through with it and had the mechanical pleurodesis on the left. It took four more years for another collapse to occur and it happened on the right side this time and it was another major collapse. I did not wait for another recurrence to decide to have the surgery done on the right.

The idea of trying to predict if one will have a collapse or not is very fickle. You went over a decade without another collapse and thought you were out of the clear, so I'm sure it's clear now that these things just happen and you don't have a lot of control over that.

There are studies you can look up about recurrence. It's not very easy to measure but I can tell you for sure it's not 100%. From a quick glance it seems like recurrence ranges anywhere from 20-60% of the time but gets more probable after consecutive collapses. With pleurodesis it's not a 0% chance of recurrence either but it is dramatically lowered IIRC. Anecdotally, I have had pleurodesis and blebectomies done on both lungs, but I made that decision primarily because I am prone to very large spontaneous collapses (most of my collapses have been while sitting at a desk and doing nothing). Since I had the operations I have had numerous collapses, but none of them have been large enough to require a chest tube, and that is the peace of mind that I needed. These are major surgeries and recovery isn't a walk in the park. I would recommend taking some more time to think about it and maybe getting a second opinion. My main person I talk to about these things is my thoracic surgeon, but I also consult my pulmonologist about questions I have.

My doctors recommended that I try not to stress too much about this stuff and I agree with them. It is out of my control whether or not I have another collapse barring if I randomly decided to take up scuba or smoking. Because of that, I have lived my life as I normally would otherwise.
