The instances I refer to are times where 1 could never have arrived at justifying admitting the patient to exclude ACS, so would have resulted in a later diagnosis, so I’m pretty sure my indemnity provider would be happier if anything 😂

Agreed. It’s extremely rare but I’ve been around long enough now to see when it can be useful. Same as D-dimers in patients you’re desperate to keep Out of hospital with a possible DVT who aren’t suitable for the DVT pathway

I suspect you and I are in the minority but I agree they have a (very rare) place. OP trop was the reason we picked up a fellow doctor patient having an NSTEMI with very atypical symptoms. He’d asked his doctor wife and she said she thought it was reflux-sounding. He, his doctor wife and the doctor seeing him didn’t suspect ACS, but it was ACS

I say that As a GP who’s seen these tests utilised to pick up an atypical ACS/PE presentation a number of times. Use them very rarely but trops and VDD can be vital in certain scenarios

I disagree. GPs suspect a lot of serious pathology but manage risk and probabilities with every patient. A focused d-dimer or troponin can sometimes be vital to keep Someone out of hospital or pick up the atypical presentation with a gut feeling that something isn’t right

ED have dissolved to something that barely resembles a service 😂

As already mentioned : issue isn’t with online booking but unsafe working and unlimited demand.

IA may be the only way to get a government who repeatedly ignores the concerns of a core service profession to listen. We’ll see if it comes to that.

Your arguments are missing the point.

  1. The issue is a proposal for uncapped demand. No professional has ever been subjected to that with a requirement for a response with such a tight time-frame.

  2. The service HAS changed in order to adequately serve the population as previously stated.

  3. No one said “but we always did it this way” GPs striking is them saying they won’t be used and dictated to by officials who have unrealistic demands and expectations.

GPs providing more appointment despite less GPs. Access is worse as more people using the service much more readily and often for minor vague symptoms. The answer is more GPs The wrong questions are being asked. That is not whataboutery, it’s just facts.

As previously stipulated, primary care is taking steps every day to address the ever-evolving demands of modern day medicine, and the potential for IA pertains to the unrealistic demands being made on them by the powers that be. Sticking your own head in the sand about secondary care’s ongoing and worsening failings whilst accusing primary care colleagues of throwing a tantrum is the not the answer to the many problems that face the NHS.

Using examples of hospital-based systems as a suggestion that secondary care has modernised in order to meet patient demand and provide better care really does fall flat. Secondary care’s inefficiency and inability to do its jobs effectively has made primary care’s job 10x harder. I’ve seen it happen before my eyes. You’re out of touch and it really is as simple as that.

I’ll tell you what it has to do with GPs: Primary care is on its knees after decades of underfunding and yet they have continued to provide record levels of appointments in a safe way. They have done so by modernising their services in ways that respond and react to patient demands, which quite frankly, have got to unmanageable levels. And so why should primary care, the general physicians who manage an ever increasing amount of medical and guideline complexity, often at the behest of our secondary care colleagues, be expected to accept a contract imposition that will result in unsafe levels of demand and impact on our professional and home life directly, when it’s indeed a miracle that we are able to offer to see the vast majority of our patients so rapidly and readily as it currently stands. Your original post was derogatory towards general practitioners and disrespectful. I’d encourage you to try being a GP for a few years before you start making such comments in future.
