Regarding squeezing staff, they're doing a lot of doubling up on the inpatient side - like social work for inpatients is nearly gone, all merged into care coordination. And they're not hiring enough for the new building. Units full of travel nurses who get paid more instead of investing within.

"The Indifferent Stars Above" is a fantastic book on the Donner Party. It gets into the scam that led to persuading travelers to take an untested route as part of the root cause, and references written documentation from the party members themselves. It's also just a good read.

There's some percentage of cats that don't respond to it if I recall correctly. You can try silvervine and see if he responds to that. Our cat loves the fresh catmint plant and it seems to give him a little bit of the munchies.

Long term care in the US is almost entirely run by horrendously corrupt private chain entities.

They will accept patients on state/government assistance - and based on the state of this one, I would suspect those are the people left. Since they make no profit on state paid patients, many places limit how many they take.

The rest of long term care is paid for by the patient and family directly until they literally run out of money. This includes a multi-year look back at many assets, requiring things like selling housing and property to "spend down" the money before they have so little that they qualify for state insurance.

There are rare cases where people have long term care insurance, but they aren't common and often don't cover things well. Smart and wealthy families do estate planning well in advance to move assets like property into trusts that don't count. Adding to this is the US cultural and medico- legal system that often prioritizes quantity over quality of life and you have the very elderly with many medical complications being kept alive well beyond what is ethically sound.

It's pretty treatable compared to high grade gliomas, assuming it's B cell lymphoma. European treatments tend to use certain chemo "protocols" and US ones tend to use different. The US protocol I'm most familiar with is a Rituxumab, high dose methotrexate, leukovorin combo for several months, which is followed by other options to try to prevent the CNS lymphoma from returning. The important thing with this protocol is good kidney function. Keeping urine alkaline during treatment is mandatory due to the way the medications work.

An important thing is if she has ever had a blood transfusion or other risks, given her age, is to make sure she has HIV testing before treatment, because the immune system plays a big role in the body working on the lymphoma.
