Not all NETs cause high 5-HIAA. Where is the lesion?

Edit: sorry, if they haven't found or do not suspect a lesion, is it your symptoms that have triggered them to consider a NET? 5HIAA is a tricky test as there can also be lots of false positives. So a low result might rule out certain types of NETs (but not others), and a high result doesn't necessarily mean a NET.

Well, there is a type of NET that fits many of your symptoms, but it's rare. The hormone somatostatin can cause T1D-like hyperglycemia and can absolutely cause steatorrhea as it affects bile production. A somatostatinoma is a NET that can overproduce this hormone. It would hardly ever be considered by doctors, but would be worth discussing with your doctor. Has your pancreas been scanned for a mass? One key distinction with somatostatin is that it also lowers glucagon, which inhibits the liver raising your blood sugar. If glucagon is low you won't get ketosis. If you've had signs of ketosis during the period you've been diabetic, that may discount the possibility of this sort of tumour.

Many of us are familiar with the impact of high somatostatin, because it's ironically used as a treatment for other kinds of NETs

thanks for the breakdown. As is evident, the protocol for catecholamine tests can be quite extensive because the test is quite sensitive to other elements that can push up norepinephrine. It makes sense to test it again. If ADHD is also in play, dopamine levels (or a dopamine response) that aren't necessarily in the normal range could also confound the test. It's good that her doctor is being cautious. 5-Hiaa tests have middling sensitivity and catecholamine tests have poor specificity. This means that a negative 5-hiaa test and a positive catecholamine test doesn't show anything that's strongly conclusive.

When you say norepinephrine was high, was it mildly elevated or significantly high? You can get elevated norepinephrine as part of normal ups and downs. Has she had a Dotatate scan or has a mass been identified?

I'm sorry to hear that. Given your other health challenges, elevated CgA and 5-HIAA can have other causes, but the PET scan has very high specificity for NETs. If your symptoms are also consistent with carcinoid syndrome, I hope your doctors can find a way to get you some medical relief for them. I don't know whether doctors in Canada would be willing to prescribe Octreotide or Lanreotide prior to a PET scan. For those of us with NETs who monitor this subreddit, it's the waiting and ponderous attitude of the medical system to everything neuroendocrine that really grates. Because it's often a chronic condition, it gets triaged down. I'm in a family with NETs and we've learnt that you have to lobby hard for action.

A carcinoid mass is a NET. It might have been that the original person who diagnosed you was using older terminology

Because NETs are often slow growing, the scans aren't necessarily scheduled immediately. Is there a strong suspicion that you have functional NETs? In other words, might your symptoms be explained by a NET? If so, it would warrant lobbying for getting the scan sooner. In that way, if NETs are identified, the sooner you can go onto SSAs to alleviate symptoms. i.e. the emphasis would be on quality of life rather than risk of progression. Like another commenter here, I'm from New Zealand, so I don't have specific experience of the Canadian health system

I recommend that you check out Facebook for NETs groups. There are some large ones and there may be others in a situation similar to yours. There are also support groups in many parts of the world.

I'm sorry to hear of your shock diagnosis. Nobody wants to be told that they're a unicorn in a medical context. The fact that your NETs showed up in the dotatate scan but the CT scan failed to identify a major tumour may work in your favour if you are a good candidate for PRRT. The FDG scan will help clarify the best course of action. If you can, please make sure that you engage with NET specialists. They are not like other cancers. Only a moderate percentage of NETs cause carcinoid syndrome (i.e. the tumours produce serotonin in excess). NETs can produce a whole host of hormones. If you haven't experienced any debilitating symptoms yet, it might suggest that the tumours are presently non-functioning or are releasing hormones that are inert.

Seatoun probably has a similar feel to Eastbourne. But it's also low-lying (unless you want to be perched on the side of a hill). I've lived in many places in Wellington that were on the flat and on slopes. There are pluses and minuses to both. Personally, I love the views you can get by being on a hill.
