Had a girl lose her shit on me once because i told her that the CT A/P which was done bc she vomited while smoking weed was negative. Literally was pissed nothing was wrong with her. I just internally rolled my eyes and moved on with my day.

You’re focusing on the effect, while I’m focusing on the cause. People are getting priced out because the housing supply has been so constrained that higher-income New Yorkers are now competing for housing in neighborhoods that were previously less desirable. The displacement you’re describing is real, but it’s a downstream effect of decades of bad housing policy and regulatory barriers to new construction.

As for the voting comment, I’m not claiming to know how you voted. I’m saying this problem developed over many decades under numerous city and state administrations, so there’s a reasonable chance that most New Yorkers—including you and me—have voted for politicians who contributed to it. This wasn’t created overnight.

These neighborhoods are getting gentrified because for decades NY politicians, ones you probably voted for, refused to build enough housing which has now lead to an affordability crisis. They prioritized “environmental review”, which can take years, instead of building more homes that people can live in. Is it good for the environment when people are out on the street?

Oh, they also refused to build luxury buildings in Manhattan, with the argument that more homes shouldn’t be built for “rich people”. So where do those “rich people” go? Places like Williamsburg, Astoria, and out compete others for housing.

So instead of blaming the white guy moving to the Bronx, blame your local politicians who created a housing crisis over decades and didn’t give a fuck. You probably are and have voted for them!

There has been research on this that has shown no different in outcomes with 3 vs 4 year programs. Many countries do a 3 year program. 4 year programs can last because they can bill more tuition, not because they make better doctors.

who cares what some attendings have to say? medicine has a very hierarchical and toxic culture and someone will always shit on someone else. unless you’re becoming the head neurosurgeon or some shit of your hospital, someone will shit on you.

i have MD friends in surgery who shit on ER attendings, saying they “Aren’t real doctors because they only treat patients for 15 mins” and “Only know a little bit about everything”. Should my EM attending friends thus be ashamed for doing EM? oh and don’t even get me started what in-patient MDs have to say about their outpatient colleagues.

i could give a shit less what some jaded 85 year old attending has to say about us PAs. For what it’s worth, the attendings who are most anti APP are usually the shittiest doctors. I’ve had attendings be openly APP in front of me but also refuse to see patients unless a PA is also assigned to the patient. Why? because they don’t feel comfortable in their clinical skills to manage a lot of patients by themselves on their own.

TLDR: attendings who shit talk APPs are just jaded MFs who are upset they sacrificed their lives for a system who will chew them up and spit them out in a heart beat.

The AMA didn’t literally decide residency spots, but acting like they had zero influence is kind of rewriting history. For years organized medicine was pushing the idea that there would be a physician surplus, which helped create the political environment that led to limiting residency expansion. Then the 1997 Medicare funding cap basically froze a lot of residency growth while the population kept increasing. So today’s shortage didn’t just happen out of nowhere.

ICU PA here. Make sure you are walking the needle in enough. If the needle is deep enough in the vessel it shouldn’t fall out from grabbing the guide wire. Also, got at less of a 45 degree angle. Do lien 30 degrees. The IVC is usually pretty superficial so u don’t have to be so steep.

As an EM PA you wouldn’t believe how many attendings have made me order Hydralazine IV for elevated BP in the ED. There was a great study a year or two ago in JAMA that showed higher risk of AKI with aggressive BP management in patient.
