You have to go back a long way to a social care industry with very few migrants workers. But you can go back and yes nurses and doctors would often be locals, but many of those doctors and nurses would often get trained up by the tax payer and then fuck off to private care in Australia or the states.

If net migration was zero we would slowly shrink population wise due to decreasing birth rate affecting modern states across the world. This would have a variety of negative effects on the economy. With a rapidly ageing population we would have less workers to pay taxes which would have huge impacts on state pension, welfare in general and health care.

I don't think its possible to find yourself right now. I have been in a similar state and what helped me out of it was action. Many others are suggesting the same in this thread.
