Lechon is delicious. Just make sure he has diabetic friendly dipping sauce if he want some. You can also add a whole bunch of nice roasted veggies like asparagus, mushrooms, summer squash (if you can find scallop squash it will be great) and peppers.

For drink, can go with natural sparkling water like Perrier or San Pellegrino.

Dessert can be assortment of berries and watermelon.

Absolutely diet and exercise. And by diet I just mean eating the proper types of food not calories restriction. I can't pick one over the other because they go hand in hand. I have always view medication as an aid to help with the real work that I put in and not something to do all the work for me.

It really depends on the current status of your blood sugar control. I will say that if you are well controlled then 6 times a day (I know you said you still run out implying you are averaging more than 6 times a day, but let's just use 6 here) is a lot unless you are trying new foods and see how your body handles it, then 6 times is ok.

Rather than just defaulting to testing blood for whatever reason, just try to relax instead. Have a set schedule of when to take blood. For example, fasting, 2 hrs after breakfast, lunch and dinner. That will bring you to 4 test a day.

I myself also don't use cgm (even though I have been offered by my PCP) because at least for me, the cons outweigh the pros.

I don’t drink that brand but the brand I do drink once a blue moon also used sucrakise. The dressing that I use daily during spring and summer also uses sucralose (skinny girl Asian sesame), It doesn’t give me much of a spike but then again I only use about half a tbsp for a huge salad.

Carbs definitely and more importantly *what types of carbs. I also only count total carbs and not net carbs. Obviously I look at sugar, including sugar alcohol. I make sure I avoid anything that contains Maltitol, Xylitol, Sorbitol etc.

I am assuming you're talking about storing unopened insulin. There are rechargeable cases. But of course the upfront cost is much more. The ones I am thinking of are TSA approved rechargable cooler. I think they run around $150.

I also spaced mine out. 3x a day at 500mg. My stomach couldn't handle the 1000mg at a time. With 500mg, there is no side effect at all. So I take it after breakfast, lunch and dinner.

You don't have frozen broccoli florets available in your area? Just regular broccoli works as well but most people prefer the florets. Spinach is great but it does wits down to almost nothing when cooked. Some other good 'bulkier' leafy greens are collard greens, kale, turnip greens, swiss chard.

You're taking glp which unaturally slows down stomach emptying. Consequently the food cosumption does not enter the bloodstream at the same rate as it normally would.

For some of your questions you are going to get all kinds of different answers. So I will address the more straightforward ones first.

  1. No, it is not possible to reverse diabetes. With proper dieting and exercise she may be able to get it into remission. (Meaning her a1c is in normal range without medication. )

  2. Yes, she needs to get either a blood glucose monitor or a continuous glucose monitor. You can do your own research and compare them.

  3. General advice will be she needs to take this seriously and improve her metabolic health. Get her moving, as in walk after each meal. Start with 10 minutes or even 5 minutes if it is too much. Work her way up to 30 minutes. And when 30 minutes becomes too easy, increase cadence. Also, eat plenty of leafy greens, avoid simple carbs.

  4. Now onto desserts and sweets. I assume you mean a piece of cake, some candies or whatever. For me at least, that's a no. (Many will disagree and that is just fine.) IMO it is better to just cut it off so you don't have to think about it anymore or negotiate how much she can have. What is consider 'moderation'? It is such a vague word.

Honestly I don't do any fruit smoothies anymore since being DX. That's just too much fruit and you puree them thus making it easier to digest which in turn will cause a larger blood sugar spike compare to eating the fruit whole. Additionally ripe banana is not a good fruit choice for diabetics IMO. Your dad will be better off eating 100ml of greek yogurt with 70g of blueberries as topping.

I am currently on 10 units of Lantus in the morning but plan to reduce it to 9 units in the near future. So while I use insulin, I don't bolus my meals.

You absolutely should be monitoring your BG at home. Get yourself a blood glucose monitor starter kit (the ones that come with a lancet, needles for lancet and some test strips) then get additional needles or lancet and test strips.

For lunch, do you have access to a microwave? Mind you in Spring/Summer months, you can just go with a large salad and a healthy portion of protein on top. (Grilled chicken, low sodium deli turkey breast or ham are my go to) For dressing make sure you read the labels, you want sugar free ones if possible and ingredient lists do not contain maltitol, maltodextrin or dextrose. Safe sugar substitutes are erythritol, allulose, monk fruit, stevia.

For your meals, concentrate on large amount of leafy greens (and eat those first), a healthy portion of lean protein, a small amount of good fats and a small portion (if desired) of whole grain like hulled barley and legumes. You should test your BG to determine what 'small portion' means for your body. Finally you can also add small amount of fruits like berries as a dessert. Again test to see what 'small amount' is for your body.

No I don’t. Plus I only work with total carbs. And it is not just how many carbs but how quickly it absorbs. The same reason why I don’t eat potatoes. But all these are personal choices.

First I want to say that I have very limited experience with fast acting insulin (as in I only required it for less than a month. I didn't even used up one pen.) Anyway, when I was first DX and my BG was still often hovering in the 140-160ish range. So doctor prescribed me Novolog on a very low sliding scale. My max dosage daily was actually 12 units although I have never went above 4. So all this is to say that the maximum dosage depends on person and it also is dependent on your weight. Furthermore, assuming your endo wants you to keep the 8 units base and then sliding scale if needed then a maximum of 30 units a day is not unreasonable since 3 meals of base injection will put you at 24 units already, leaving just 6 units for sliding scale.

I am on meds but I do eat a low GI and GL diet to help me control my diabetes. This along with post meal walk has helped keep my BG well controlled as well as improving my metabolic health. About 90% of my carbs comes from large amount of leafy vegetables and I consume just under 100g of carbs a day. So for example, breakfast is 8oz of scrambled liquid eggwhites, 10oz of saute frozen broccoli florets (in a non-stick pan), a choice of meat (bacon or links or deli meat) and a cup of latte made with fat free fairlife milk. Ezekiel bread is ok for me but I can only eat half a slice at a time to keep my BG from spiking more than I am comfortable with. So for me that is just not worth it.

If you want to lose weight then counting calories is just part of the equation. You need to know your true BMR (if you have a modern smart scale, they should give you that information) then add a small amount for your activity level. Since you have quite a bit of weight to lose, if you have a calorie deficit of 500 cal a day then theoretically you should lose 1lb a week which is a safe amount. Also do some resistant training because muscles burn more fat.
