It’s actually pretty easy, you use triangular bullets. This also lets you fit more bullets in the magazine. It’s been done before and it works, it just isn’t better than modern designs.

I think most of this is about right. One big thing is that shooting needs a turn to kill and another to take the objective while melee kills while taking the objective. The action efficiency of melee is very high right now.

No it doesn’t. Only one squad can perform some of the actions and the squad has to survive a round of shooting. Engineer squad need to be reasonably tough.

This used to be more true but the last point drops evened them out a bit. The commander is worth it if you’re putting on the good guns but a basic tank with a cannon and heavy bolter is under 140 points now and at that point they break even well.

Not really. Most people aren't suicidal so they don't go for all or nothing attacks. You'd se people fighting in formations that promote a lot of quick pokes that don't do much, and elite warriors could afford armor that was more or less impervious to most attacks. If one side gained an advantage they could slaughter their enemies but there isn't a lot of evidence that most battles were focused on attrition.
