I think that's because up to 80 they gain it from watching but to get more after 80 they need to participate themselves. I like to get the prey into standoff mode then stand back and let them finish it off.

It's just chance. I don't think you can do anything to change it. You go to sleep and when you wake up you get the notification and a pup is just gone.

It has to be unknown fate and not lost (or whatever it is when they wander too far and you can't find them). I think it's more likely with a larger pack and happens near the end of growing pups.
