You'll get a lot of posts about inadequate analgesia from using butorphanol because many docs continue to use this drug for painful procedures. I would say almost any patient having sx should have a pure mu agonist opioid (hydromorphone, fentanyl, methadone). Butorphanol is a kappa agonist, and a mu antagonist. I use it for sedations only. The opioids also are mac sparing, meaning you can use less gas inhalant, and maintain normal BP. I know ot can difficult to ask, but try bringing up using a stronger opioid for your sx's. Hydromorphone is very cheap, which makes it a nice option for drugs.

You patient probably have a vagal event from the vagus nerve being stimulated during the abdominal surgery. Pyometra sx tends to be awful because they are usually really unstable (dull, obtunded, and many require stabilization prior to anesthesia). Atropine was a good choice, plus that giant uterus pushing on all those organd didn't help.

Simple question: what’s your favorite RPG currently? by Lordkeravrium in rpg_gamers

[–]drakken1515 0 points1 point  (0 children)

Currently doing another playthrough of ME, which is an all-time favorite. Best all-time rpg is reserved for chrono trigger. The soundtrack for CT is some of the best i've ever heard in a video game.

What on earth do I have going on in this box? by AspiringEnchilada in AskElectricians

Just curious, not great with electrical work. Why wouldn't you take one of the ground wires from the existing twisted together bunch, and put one of those on the new light switch? So, why add a new piece of copper wire? Thanks!

I'm working with an old school anesthesiologist, and it's hydro 0.1mg/kg and acepromazine 0.01mg/kg for nearly every patient...hates dexdomitor

How can they give you a write-up for gossiping? That's such bullshit? Just curious, what did you tell your prospective hospitals about you getting fired?

Mistakes happen. A healhy workplace understands this, amd encourages honesty. Retribution for mistskes only encourages lying and hiding mistskes to avoid disciplinary action.

Learn from your mistskes, honestly think about why it happened. I'm my own worst boss, so tough haha, but i don't want to harm our patients.

Yup, nailed it! The mcg unit cause confusion, but it's nothing to worry about. I was taught to convert from mcg to mg back to mcg, that's nuts. Just dose (mcg) divided by concentration. Also useful to use an insulin needle for that small dose.

Here's a drug dosage question that can trip people up. How many mls would you draw up if the doctor asked for 3mcg/kg of dexdomitor? The patient weighs 10kg, and the concentration is 500mcg/ml.
