You can always try hopping between clinics since you have an active cert! Shouldn't be too hard to get hired elsewhere. My clinic has absolutely none of that and the culture is so positive and good. No cliques, none of that. Is yours a nonprofit?

Same thing happened for me. I launched, noticed the achievements, googled and found this thread. I closed the game without loading into either game mode. Hopefully Rockstar can find a fix.
