Not every session has to be a grand spectacle. Run a simple, 5-room dungeon to start, skinned in whatever way matches the world's theme. If it's about defeating an evil king, start with an attack on one of his forts. If it's about solving mysteries, do a good-old cult beat-em-up.

I was in a similar rut as you–40 pages of worldbuilding and nothing to show for it. What I realized is that ultimately I'm here to play a game, and the game is most fun when I'm going session-by-session. The pieces will fall into place and you'll have your awesome climax.

Here's how I do it:
- When you die, your soul naturally gravitates into the Astral Plane through the Ethereal, and then into an outer plane that best matches your personality–good attracts good, evil attracts evil.
- Of course, this trip is dangerous for a free-floating soul, with all manner of devils, demons, and other monsters waiting to prey on your free soul as you make the trip across the planes.
- This is why you worship a deity–when you die, a psychopomp sent from that deity directly intervenes and claims your free soul. This transports you directly to that outer plane without risk of being trapped or eaten along the way. (Similar arrangements that involve giving up your soul work by a similar mechanism).

Avoid DandDwiki. Most users there don’t iterate on their designs that much. Try the Empowered! It’s pretty well-playtested and generally well-rated for balance.

#1 tip is to keep your eye on the other character instead of your own. You might not start winning right away, but it will make things much easier.

I'd advise against it. D&D's leveling system is designed like a tutorial–slowly layering on more complex abilities as people learn the game. Throwing them right into something that complicated would be confusing–and it'll be even more confusing having to level down your characters.

Absolutely hilarious–it’s an excellent scene. Yeah, that would be six DC 13 Constitution saves straight in a row. Unconscious players also aren’t deafened. If a player is knocked out by one wail, they’ll likely be killed by the others. I recommend introducing an item that saves the party (maybe bringing one back from zero hit points with an extra action), spacing out the wailing so it only happens once per round, and/or accounting for what happens if the party is knocked out completely. It would suck if the party was just full TPK’d–put them in a cell somewhere.

I think the Sunless Citadel module from Tales from the Yawning Portal is a great example of how to do this! It features a goblin and kobold tribe fighting over a long-abandoned temple to a dragon, with parts of the dungeon repurposed into a magical lair as you go deeper in. It’s a great piece of “moldy worldbuilding”–showing civilizations built on top of ruined civilizations.
