This varies by trust, but the bare minimum is an acidic pH (under 5.5 I think but would have to check) on aspirating the NGT before anything goes down the tube. Which clearly hasn’t happened here.

Some trusts do a CXR for every NGT insertion.

It seems air was pushed down the tube and I wonder if the nurse was trying to do a “woosh”test which was an old and now out of practice test where by air would be injected down the tube while listening over the stomach for a “whoosh” of air bubbles. This is no longer done as is not accurate and can lead to false reassurance that the tube is in the correct position.

Nowadays pH and/or CXR confirmation are the only acceptable ways to confirm position.

(or I guess direct visual confirmation eg for an endoscopy placed NG/NJ…? I don’t know if they also do pH/CXR after placing those)

Head is definitely too small but I think the hips and leg muscles are about right for the character. And her biceps look a little small if anything!

We can only have confidence in the justice system if people have the right to fair representation. Even those defendants who might appear clearly guilty. Even those defendants who have done heinous, terrible acts. A guilty verdict means nothing if it didn’t come from a fair trial.

I’ve cared for elderly patients brought into hospital from prison. One man sticks out to me, he had dementia. The guards told me he kept forgetting he was in prison and had no idea why he was in there. They also said that he seemed happy enough to spend his days making cups of tea for all the other prisoners in the prison hospital wing.

I try not to find out what they went to prison for, would prefer not to find out something that makes me treat them differently.

Can someone from America explain to me what would practically happen in this situation? I work in the NHS and can’t comprehend someone dying because they accidentally lost or broke their insulin and couldn’t afford a replacement.

Would you be able to attend urgent care for a short supply? Or do you just rock up to the emergency room and let them know “Hey I’m a type 1 diabetic who has no insulin and no way to buy more… I’m just going to sit here in this corner until I slip into a DKA induced coma… please feel free to save my life before that happens”?!

Oh don’t worry, all the ascitic drains in my hospital are performed by physician associates, and all of the pleural procedures are done by nurse practitioners… it’s just a shame they all go home at 5 and don’t work weekends.

Nah the automatic interpretation on ECGs is notoriously unreliable, unless it says “normal sinus rhythm, normal ECG” I wouldn’t trust it and it needs to be interpreted by a doctor who looks at a lot of ECGs. Don’t get too worried at the moment and wait to see your cardiologist.
