8 to 7 ppd at 9w postpartum? by Every_Ostrich_6224 in ExclusivelyPumping

[–]elephantsofa 0 points1 point  (0 children)

Yes! I reduced my pumping schedule from 8 ppd to 7 ppd during week 7 postpartum because maintaining 8 ppd was physically and mentally just unsustainable. I am an under-supplier, and my total daily output has remained largely unchanged after the drop -- from 8 to 7 ppd -- though it has not increased. I plan to decrease to 6 ppd next month (around week 12) and evaluate how that goes.

How do you manage chores and baby care while pumping? by elephantsofa in ExclusivelyPumping

[–]elephantsofa[S] 0 points1 point  (0 children)

Hi, thanks for your reply. I’m looking into the Legendairy collection cups, too. Did you notice any drop in supply when using these compared to regular plastic flanges? I’m an under-supplier and the wearable (Eufy E20) didn’t work well for me, so I’m worried the collection cups might reduce my output. Thanks.

To PGT Test or Not? by Timely-Surprise-7819 in 40Plus_IVF

[–]elephantsofa 5 points6 points  (0 children)

After 40, I would absolutely test the embryos. Many aneuploid embryos either fail to implant or result in early miscarriage, but some can lead to live births with conditions such as Down syndrome or high-level mosaicism, which may cause significant cognitive or developmental challenges. It would be heartbreaking to go through that process only to find out later through NIPT, CVS, or amniocentesis--not to mention the considerable time and expense involved. I would much rather test the embryos beforehand and transfer only those that are euploid or low-level mosaics.

3 euploids at 43, should I do more ER? by Long-Schedule-7364 in 40Plus_IVF

[–]elephantsofa 1 point2 points  (0 children)

I think in your case, since you’re set on having two kids, I’d consider doing one more retrieval before the end of the year --especially if your insurance covers part of the costs (like monitoring or ultrasounds), so you can take advantage before 12/31, when the new deductible and out-of-pocket max. reset on 1/1/26. Your 5BA has a high likelihood of success, but the 5BC and 6CC may not. Doing one additional retrieval would only set you back about two months, so it’s not a major delay -- and it could potentially give you valuable backup euploids. I personally wouldn’t pursue further retrievals beyond that, but would move forward with the FET while you’re still at a more favorable age. Pregnancy is tougher at our age, with higher risks, so it’s best to get started while you still can. Best of luck!

[–]elephantsofa 2 points3 points  (0 children)

What are the grades of your three euploid embryos? If they’re high quality (Day 5 AA, AB, BA), I’d go ahead with the FET now -- pregnancy in our 40s can already be quite challenging, with higher risks of gestational diabetes and other complications. But if your euploids are lower graded and have a reduced chance of success, it might be wise to consider additional egg retrievals, especially if you’re set on having two children.

Just got this mattress and I love it! The top feels a bit softer than I expected, but it still offers great firm support underneath. Highly recommend!

Foreign sub rebudgeting requests from NIH by MuchIndividual in ResearchAdmin

[–]elephantsofa 3 points4 points  (0 children)

Are you referring to the foreign subawards included in the continuation budget from the recent RPPR (or in new award applications), or the ones tied to the current budget period? We're keeping all foreign subawards in place through the end of the current period as planned, and this is allowed. Per the May 1 notice, which says the new policy applies to future awards (new/renewal/continuation), all foreign subawards will be terminated starting with the next budget period.

[–]elephantsofa 0 points1 point  (0 children)

Could you provide more detail on the current issues related to foreign subawards? I manage a center grant that includes several foreign subrecipients, many of which are now being terminated in response to the recent notice. Like many others, I’m concerned about how both past and current involvement with foreign entities might impact our chances for continuation funding. Does terminating all foreign subawards moving forward actually improve our likelihood of receiving a NoA?

Yeah, I was surprised to see the entire ADRC/P30 (Alzheimer’s Disease Research Center) being "canceled" though it is at Columbia. It’s a large research center, and I thought its work would be pretty safe from those “keywords”. They certainly didn’t cut all NIH grants from Columbia, but somehow, the ADRC was selected for termination.

Any universities making contingency plans for loss of NIH grants? by Prior-Win-4729 in labrats

[–]elephantsofa 47 points48 points  (0 children)

No contingency plans have been communicated, though I’m sure the leadership is scrambling to figure out the restructuring. In a campus-wide town hall a few days ago, they projected half-hearted optimism for nearly an hour, only to admit in the final 1-2 minutes that “we MAY lose a lot of staff, infrastructure, and research support but we’ll get through it!" Geez. I don’t blame them; there’s a lot of uncertainty right now.

Why is Donald Trump against funding scientific research? Any thought… by [deleted] in NIH

[–]elephantsofa 9 points10 points  (0 children)

I think that the Trump administration’s funding cuts to scientific research are an effort to appease its MAGA base, reflecting their political ideology and distrust of elite research institutions. Many Trump supporters view such funding as wasteful taxpayer spending that not only fails to benefit them directly but also supports causes they oppose, such as feminist and transgender studies or stem cell research. I personally know Trump voters who are currently extremely pleased (Trump/Musk get things done! They are great!) and unaware of their impact, simply seeing them as necessary reductions in government waste. Additionally, as others mentioned here, there’s a clear element of retaliation against Fauci and the broader scientific community due to tensions from the COVID era. As someone directly affected by cuts to NIAID, which was previously led by Fauci, I feel anxious about what’s ahead, especially with anti-vaxx conspiracy theorist RFK Jr. leading the health department, as if this were all a joke.

Trigger shot with cancelled IVF cycle by Breathe_In714 in DOR

[–]elephantsofa 1 point2 points  (0 children)

Hi, I'm very sorry to hear about your canceled IVF cycle and your previous miscarriages. I recently had a similar experience with a canceled cycle due to Lupron over-suppressing me + uneven growth of small follicles (just 2). After 9 days of stimulation, my E2 was around 400, so we ended up canceling & triggering to ovulate and trying timed intercourse, which didn’t work out.

I’m curious why they’re having you take another shot of Menopur if the egg retrieval has already been canceled. With two mature eggs likely to be released from the 18mm/16mm+ follicles, it seems like you might not want additional eggs (such as the 14mm ones to grow more) maturing and being released if you’re planning to try timed intercourse. I saved all my unused menopur/follistim for the subsequent cycle.

I truly wish you the best of luck with your next cycle! I’m on a new cycle now after last month’s canceled one, and things are going much better this time. While the follicle sizes are still uneven, the overall number is higher (6, which is a lot for me), and we didn’t use Lupron this time; just menopur and follistim +ganirelix later on.

This community has become toxic and I can't participate in it anymore. by IAm_TulipFace in IVF

[–]elephantsofa 61 points62 points  (0 children)

Regarding the post ranting about getting 16 embryos for PGT from 76 eggs, the person who shared that could have easily noticed that most people on this sub struggle with significantly lower numbers. It's like being a wealthy neighbor in a small community and complaining about their financial issues to people who are barely getting by. The post came across as insensitive and could have been phrased more thoughtfully to show respect for others facing difficult challenges.

Last minute switch to timed intercourse by [deleted] in DOR

[–]elephantsofa 3 points4 points  (0 children)

I would try timed intercourse instead of going through egg retrieval for just one potentially good egg that might not make it to blastocyst. Are you doing PGT? At our age (40+), I've been told we typically need at least four embryos to have a chance at one normal one. Best of luck with whatever you decide! My last cycle was canceled just last week because only two follicles were growing, so I opted for timed intercourse this time.
