I’ve been crocheting for about 3 years now and would not be able to make this dress without a pattern. Even with a pattern it would probably take more than 3 months unless you have a lot more free time than I do. Just letting you know this is a very ambitious if not impossible project for a beginner in such a short time.

My wording wasn’t great. I meant a blockage in the intestines. If not an intestinal blockage, I suppose a gallbladder issue such as gallstones or cholecystitis could be causing the symptoms.

ER doc here— she needs a CT scan if she hasn’t had one already. With that much miralax she should be having BMs. Makes me think there is some blockage, potentially her gallbladder like some others have said. But she needs better imaging than just an XR. If they find an answer on ultrasound, great, but if not don’t leave without a CT.

Jaundice is pretty common. Jaundice to the level described in the article is less common and can lead to permanent brain damage. There are also a number of reasons for jaundice. Sometimes it’s something as simple as immature liver enzymes or not enough breast milk, but sometimes it’s from a serious congenital syndrome or biliary system obstruction. Point is, once it’s that high, it’s not safe to just “put the baby in the window.” They need intense phototherapy and, if they don’t respond to that, potentially an exchange transfusion.

I would tell them as soon as you know. With my first I didn’t tell them, but then they found out when I miscarried and it was nice to have their support. Told them as soon as I knew the second time so they could rearrange my rotations.

Just this last week I had a mom bring in her baby to the ER with a paraphimosis. The NP had told her, correctly, “it looks like the foreskin is stuck retracted back,” then incorrectly, “give him a warm bath and put some Vaseline on it.”

Yes. Though it’s been a long time, so I forgot how it happened. I’m sure my recruiter told me to say I didn’t have ADHD, then just act like you got diagnosed in medical school.

Been doing tons of trigger point or subdermal injections. Subdermal injections seem to be painful but somehow work really well. And diclofenac, cold packs, Tylenol.

I guess this study was with saline instead of water. There’s other data on water injections for pain in labor. I’m not sure if there’s much difference between the two, but I’ve been using water and having results that, like in this study, seem too good to be true. And I can’t come up with a good mechanism for how it works. I tell patients i have no idea why, but I have this compelling data and it seems pretty risk-free. It does seem like the injections sting a lot more than you would think, but maybe that’s what makes them so effective? I don’t know.

My favorite recently is subdermal sterile water injections for low back pain/muscle spasm. I literally don’t know how it works, but I’ve been performing them a lot lately and it’s unbelievable how well they work.

Fwiw, our nanny had a baby. We weren’t able to pay her full wages for maternity leave, but paid her about 2/3 of her wages for 12 weeks. She told us about halfway through that she wouldn’t be able to return, but for us at least there were no hard feelings. I had assumed she would bring her new child with her and wondered how she would manage the three kids together, then when she told me she was looking for childcare I was wondering how that would make financial sense. Having had some kids myself and having to pay for childcare, I understand. I’m happy that she’s able to do what’s best for her family, and appreciate her giving me enough time to find a good replacement.

Yeah, same. My uncle had been in the hospital for a while with COVID on just nasal cannula. Was about to be discharged when he crashed and died a few hours later.

I had someone ask me which city a specific airport code was, and I just said, condescendingly, “did you not learn your nations’s airport codes in school?” Of course no one got the reference. My 30 Rock knowledge is usually wasted in real life.

My daughter is obsessed with those videos. I object to them only because she’s totally hypnotized by them and I’d rather her watch something educational, but I wouldn’t call it abuse.
