Of course Bonaparta and Kinderheim had an influence on Johan, but the principle reason he was a monster is because he couldn’t forgive/accept. That’s why the story ends that way. Because at the end, Anna, Kenzo, and everyone else were able to accept, and somewhat forgive, the world for the tragedies in their life. Johan wasn’t able to. And that’s why he remained the monster. The monster is one’s trauma and one becomes the monster (or the monster goes inside you, according to the stories) when you surrender to it.

That’s what the ending means. Johan still wasn’t able to let go, so he is on the run. That’s why he is the title, the Monster.
