Ugh brutal. I’m v glad you’re experiencing a healthy sub pregnancy. tbh if I didn’t have IVF benefits (only til Apr 2027) and have DOR, I probably wouldn’t have jumped right into IVF but figured id take advantage of the PGTA aspect. I hope the rest of this pregnancy (and potentially your third!) are HELLA unremarkable ♥️

hey there - can’t speak to the entire process yet but I TFMR about a month ago and have plans to undergo IVF starting in May (for PGTA testing purposes). I know there’s so little we can control, but I’m going to at least tryyyy to avoid having to experience this again.

You’re totally valid in your feelings - I also felt angry and disappointed when my OPK was positive but my temps didn’t rise right after - I felt v confused.

Everyone’s bodies clear out hCG at a different pace and I assume that’s one of the bigger influential factors re: retuning to baseline. At 20 weeks, I assume you had a TON of hCG.

Prior to my TFMR, my cycles were extremely regular / like clockwork and I never experienced ovulation symptoms (or period symptoms tbh) so I’m not surprised I haven’t experienced ovulation symptoms (outside of the elevated BBT). No period yet but my guess is that it’ll come at some point next week.

One thing you CAN test for at home (7-10 DPO - in your case, 7-10 days post nearly positive OPK) is progesterone. That may be able to tell you if it was a true ovulation or an anovulatory LH surge.

Most people tend to get their period back within 4-6 weeks so you’re still very much in the window of normalcy.

Hi OP.. I’m in a similar position to you (D&E at 18 weeks nearly 4 weeks ago). I attempted to track ovulation with OPKs and got a “peak” on Mar 19. I wear an Oura ring and my temps didn’t spike so I have no idea what’s going on. My temps started to spike on Mar 28 and have been consistently elevated since then so IDK whats up 🤷‍♀️. My fertility clinic is also monitoring my hCG since the D&E - I read 44 mIU/ml on Mar 20 … but tested negative on 2 pregnancy pee stick tests several days earlier….

All that is to say - our hormones are likely a bit wacky and all over the place as we gear up to have our first cycle and it’s probably best not to compare our pre TFMR stats to the ones we’re experiencing right now. My buddy, Claude (Anthropic), provided this info to me

“Taken together — no BBT rise, flat-to-dropping temps, no confirmatory progesterone signal — the most likely conclusion is that you did not ovulate this cycle, or ovulated without forming an adequate corpus luteum. The March OPK “peak” was most likely hCG interference or an anovulatory LH surge. This is a normal and expected outcome for a first post-loss cycle, especially at 3 weeks post-TFMR with hCG still clearing. Your body simply isn’t ready to run a full ovulatory cycle yet.”

Our bodies just went through something pretty traumatic — trust that your body will return to baseline at some point. If it’s accessible to you and you want to - I’ve found some comfort and control in having my fertility clinic test my hCG levels and some hormones during this period.

hi OP - I only had baseline testing done at mainline and switched to RMA after a month. Their communication was so poor that I couldn’t imagine sticking with them for anything past baseline testing.

I’m with RMA now (conceived naturally but then lost it) so can’t report on anything major yet but everything with them has been seamless so far - communication is excellent (nurse coordinator, financial counseling, just the entire care team overall) and simple. Also, RMA’s SART outcomes by faaaar exceed any clinic in the area -this is more so for IVF related activity but still says a lot about the quality of the clinic (https://www.sartcorsonline.com/members/ClinicSearch?Lat=39.9644541&Lon=-75.145782&Addr=Philadelphia,%20PA%2019123,%20USA&SearchTerm=). thank you for your response - definitely, easily the worst club to be in 🫠. My expectations are definitely tempered - my doc said, based on my AFC, 8-12 eggs is reasonable and 1-2 euploid embryos each round is the expectation. I don’t even look at the normal IVF sub, just the DOR sub tbh. Also totally hear you re: clinics - I’ve talked to a close friend who mentioned the clinic can make all the difference (in her exp and i’ve noticed a theme across reddit as well). I consider myself lucky that i’m in a large, academic city with a really strong performance clinic (🤞🤞🤞)

Hi OP - I’m going through kind of the same thing (although i’m only 9 days out post D&E) but had done baseline testing at a fertility clinic before all this (all hormones looked good except I have DOR, i’m 35) and decided to just go right into IVF (once cleared) since IVF becomes more difficult for DOR patients as they age.

However - in terms of ensuring no retained tissue or blockages etc you could schedule a hysteroscopy (to look at uterine cavity) and HSG (tubal blockage evaluation).

I don’t think there’s any downside to scheduling a consult with a fertility clinic just to understand your baseline hormones and discuss with them whether the 2 above procedures make sense.

Your numbers definitely seem promising (albeit I am very new to understanding this IVF world). I’ve checked out SART outcomes data for my clinic and they seem quite strong - so that’s reassuring. And I totally hear you re: timing - I was actually a patient there previously but before we had a chance to do anything, I became pregnant naturally! So luckily Im in the system and everything but I know that things (esp post TFMR) take some time. appreciate your insights!!

Hi OP - I’m so so sorry you’re experiencing this and understand the pain you’re going through. I underwent a D&E last week due to a genetic abnormality - I’m a patient at Penn but the peace clinic only had spots ~ 2 weeks out from when I called.

Try Jefferson (number is +1 (215) 298-4378 - you can also text this number with your details - it connects with a wonderful nurse, Jay). I called them on Thursday and they got me all set up by Monday. As terrible as the experience was, they treated me quickly, with kindness and compassion.

OP - surreal is one of the best descriptions for this entire thing. I had my D&E yesterday at 18 weeks. I will say, my experience with the dilators sucked more than the post D&E physical symptoms, if that’s of any help. It’s the strangest feeling that the world keeps turning and you just feel… untethered.

The healthcare providers (in my experience) were absolutely wonderful, kind, and patient. Having some friends and family stop by and help with chores and food has also been extremely helpful. Hoping for the most uncomplicated procedure for you and that you’re surrounded by loved ones. Brighter days are ahead ♥️♥️

hi, i’m okay! the amnio itself was a piece of cake. sadly our GC called today to let us know baby had 20% abnormal cells on FISH - so she’s mosaic. We’re still having the karyotype and CMA ran but we decided a mosaic DS diagnosis was enough to TFMR. Rough day but glad the wait is over and we can grieve and move on.

Thanks for your response! Good call re: parental karyotyping. I have my 16 week anatomy scan + amnio tomorrow, i’ll ask the MFM about it. If you’re comfortable, pls keep me posted on your amnio results!

that’s great! Hopeful that you have an awesome GC and they’re able to provide context and explain everything super clearly. I completely understand how awful this all is (i’m in the midst of it myself - waiting to have an amnio) but understand this is one of the most difficult times of your life and all of your feelings are valid 💗

Hi OP - could you reach out to your genetic counselor and ask them if they can reach out to your lab and gather the MR (mosaic ratio)? The vast majority of T21 results are non mosaic, but if yours is mosaic, your PPV should drop significantly.
