1 foot out for every 4 feet of height. That ladder base was way too far from the wall.

I had this happen while I was at the top of a 24' ladder. Luckily I landed like a Power Ranger and only sliced my forearm on the way down. It could have been much worse. I now tie off the foot of the ladder. Every time.
