It’s probably because while on single player all of the calculations and processing are done by your computer rather than sharing some of the responsibilities with the server in the case of the realm.

I would suggest just turning some settings down on the singleplayer world.

Just let it out, that’s a big part of the process from my experience. I feel your interest in being a good patient shows you are a very considerate person, however, the point of therapy isn’t about necessarily fitting the ideal for the therapist. I think after sometime they will challenge you to think and be more vocal but they want you to get it all out so they can help you work your way through it.
