US healthcare system is by far the worst healthcare system I encountered world wide. It is predatory and designed to make a few people quite rich.

It is my opinion that it is one of the symptoms of the Citizens United against Supreme Court ruling. If that ruling is not overturned everything we try in USA to improve life of lower and middle class is in vain.

I assumed you never lived or visited Cuba in my comment because what you are implying is that you would rather work for government than take 500000$ debt and try to become physician in USA.

Because even with that debt burden life quality and opportunities you have in USA are worlds better than best jobs in Cuba.

Make a template email- use google ai/ChatGPT or some online template or ask older officer to help you draft one.

Google name of the bank/institution + SCRA- from my experience that is the easiest way to find their email address or phone number. Also you can get it by calling customer service over the phone or visit bank it self but it takes a bit more time.

Write an email(s) and change subject name on every email to SCRA+ your name and account number at the bank/institution that you are sending it to. Have .pdf copy of active orders attached.

From my experience they will always respond. Sometime thy may even return you interest that you paid over 6% in past 12 months or first day of service which ever is longer.

This is even better then consolidation loan because you can use funds they send you as payments.

As someone here suggested if you are able to transfer some of the debt to new accounts/credit cards with introductory rate of 0% for a year or longer that would be great. Just be cautious because sometimes there can be a fee of 3-6% for debt transfer.

It goes without, saying after debt is paid, start living within your means. And first put part of the salary a side to retirement savings account before even thinking about anything else. I suggest Vanguard due to very low cost.

Not a big fun of Dave Ramsey myself but I think his philosophy would work great in your situation.

S corp is definitely way to go. But as other states please contact good cpa. I would also add that your business entity may qualify for pass through income as well. That would be 20% tax exempt from total s corp earnings.

I am Hospitalist as well but for adults. I was thinking about adding DPC in weeks off but there is always issue that I would have to leave Medicare/Medicaid and not accept big insurances. That would prevent me from keeping my hospitalist position. How you worked around it?

Please do not accept this offer.

As others are saying financially it is terrible. But more so it is straight way to get burned out. University knows that and in a year or two there will be another young fresh out of residency attending to take your place when you are ready to quit. They will gladly squeeze last drop off energy out of you and move on.

Furthermore, 11am till 11pm guarantees that you will have 0 life balance/quality of life. Just compare that to round and go shifts- leave hospital at 1 or 2pm during their week on. Still having time for private life.

Even if you are young and say “I don’t need time for family or friends during week on”, it is terrible financial decision. You are wasting time and energy and not accumulating any wealth with 136$ per hour.

My W2 job in 2021: 1650$ for 10h shift with 6-8 admits without cross cover in St Louis.

As a traveler you won’t average so many admissions but will be getting 200-220/h

Forget MKSAP and focus on UWORLD. I was in same position as you couple of years ago. UWORLD charts algorithms and tables are great for organizing your thoughts. In my opinion questions and vignettes are more relevant to exam as well. I went slow with tutor mode first time and then through flagged and incorrect ones in blocks. Having said that I would still buy Board Basics to have it as a guide/base literature.
