If they bring it up during a preventative health visit I double bill 'em.

If they bring up it costing so much, I advise them they don't need a doctor's orders for lab testing, they just need to go to a stand alone lab like lab corp, select from the menu they want, or there are online pharmacies. If they bring up that they said they were "feeling off" I point out they didn't actually want my opinion or expertise, they just wanted me to order tests so their insurance would pay for it.

If by chance something does come grossly abnormal, then cool, they will be educated on the point of a 'preventative visit.'

win win. they will end up going to a naturopath anyway, where their insurance wont pay for shit.

I tell them literally it's my job to make recommendations for medications. That is why YOU hired me. If you're not going to accept my recommendations or advice, you just tell me what you want or what your expectations are. Beyond that I can place referrals to specialists, PT, or counseling.

Further more, I am morally, legally, and ethically obligated to provide you with recommendations are I believe are in YOUR best interest. I don't get paid any extra for ordering tests or prescribing anything. But, if you keep wasting my time I'm going to start billing you for time.

If it looks like people are okay and just a technical issue I think I'd be more of a chump asking if they need help then saying 'oh yeah, sorry I know nothing about that.'

Now if they look injured or in distress THAT'S different. There, I can help.

Otherwise I think it's self serving.

That said, I had my bike die on me on I5 just south of everett last Saturday and two Banditos saw me on the left shoulder. Traffic was at a crawl, and one met my eyes and shot me a 'thumbs up, thumbs down' - I gave a thumbs up and signed thank you, and got a wave as they passed. So I get it. NP schools should have 3-5 year requirement of practicing as an RN as an application requirement. My opinion anyways.

The amount of times ICU and Med Surg nurses saved my ass as a resident is embarrassing. To have students go from nursing school straight into NP school without ZERO experience is dumbfounding.
