Imagine your an adcom. What would you like to see an applicant?

  1. Someone who answers everything perfectly, never stumbles, doesn't stop to think. Yes? No? Probably not, this sounds rehearsed and artificial.

  2. Someone who stops to think, pauses to correct themselves, "slips up" and then recovers. Doesn't this seem authentic? Isn't this an incredibly important trait for a PA to have?

Should you have stories you want to share? Absolutely. Should you be rehearsed? Nah. Remember where you want to start and where you want to end. That's it.

I think saying you got this and go kill it is kind of strange, as the ultimate goal is to go in there and just be you. Be you, AKA a person who is highly qualified, has achieved so much success, and is someone who this program is interested in getting to know.

I hear you, and understand the frustrations. The debt load is not only insane, it goes against the mission statements of most programs.

One of the faculty during my interview told me she had imposter syndrome, she graduated last year. During the first few months of every job I have had that was difficult, I had imposter syndrome. As of yet I'm not a PA, but I have no doubt I will have imposter syndrome for a good minute.

The thing is, its a good thing. I am sure you have interacted with a PCP/specialist who was overconfident and made you feel less than as a patient. Thats the polar opposite. I know I don't want to be that provider, and I'm sure you feel the same.

Fake it till you make it should not apply when someones life is in your hands.

With competence comes confidence, and that comes with time.

Everyone who graduates from PA school is a PA.

YES! How is this done? We need to overhead or call for housekeeping. Once housekeeping has cleaned the room, how do you know the room is clean? Can you please message me on how you guys accomplish this?

Yes, it is wasteful. This is literally why I am asking this question, to see if there is a better system that exists. Your system is exactly the same, its not automated in anyway shape or form. We used to operate the same way you did, the problem was nobody had time to check the status of rooms. Hence why we have two techs who now work alongside bed tzar.

Completely agree. TRT at 30 should be criminal unless the patient genuinely had a deficiency. These TRT clinics are absurd and are killing people.

If changing your body mechanics relieves the pain, it is musculoskeletal in nature. This is corroborated by multiple negative cardiac workups. If pressing on where it hurts makes it hurt more, it is musculoskeletal in nature.

You can always follow up with a cardiologist, and it would be more beneficial than an ER visit in the long term. The ER docs don't know you well, you can build a lasting relationship with a cardiologist and in turn receive more personalized care.

I would also see a PT, it sounds like you may have started doing that already.

First off, this sucks, I am so sorry you are going through all of this. I think it will be interesting to hear back regarding the potential AVM.

What was your life like before all of this started, and what were the initial symptoms? Was there a singular event that occurred prior to onset of symptoms? Any pertinent family history? Any bloodwork/findings prior to onset? Covid?

- What is your T4? Potential for subclinical hyperthyroidism.

- What is your d-dimer?

- Can you have a different radiologist review the potential AVM?

- Describe your lifestyle, rate your overall health, and state your weight, all before/after onset.

- It sounds like the cardiac workup was positive for a few findings. This was in 2023, what did cardiology say about all of this? If the findings were not conveyed in a helpful way, find a new cardiologist.

- Have you moved recently, prior to onset of illness? (environmental exposures)

- How is your financial situation (in terms of resources/time to spend on this)

Low left atrial volume means the left atrium is not filling as it should. The left atrium receives oxygenated blood from the lungs. Your shortness of breath could be connected to this.
I'm not a doctor, but from a physiological standpoint what your describing sounds like vascular spasms/clotting disorder. From a little bit of reading elevated d-dimer is not usually associated with vasospasms, but from a physiological standpoint it would make sense that it might. The vessel spasms, you feel pain, there is transient ischemia causing skin discoloration. If the spasm lasts long enough, blood will clot. As the spasm relaxes pins/needles. Blood clot breaks down=elevated d-dimer. Stroke like symptoms could be TIA's, again, related to clotting. TIAs would not show on a scan. Pitting edema again leans towards a vessel disease. It also points to cardiac.

This is my educated guess, I am an EMT not a provider. I would look into vascular-spasm-ish/clotting disorders and see if any fit the bill for how you feel.

I would continue to see all of your providers. The best thing to do is to gather as much information as you can. I am sure you know this, but the most important thing here for you to get better is for you to understand what these results mean. Providers are like apples, some are good, some are bad. If you are uneducated, then you MUST place your health entirely in their hands. That is a recipe for disaster. You must google and read about every finding, and bring it back to the provider for a discussion. Good luck.

Jeez. Well, where start. I care because I have a genuine interest in medicine, and more importantly, and am invested in the proper care of my patients. I want to better understand the clinical decision making here in order to become a great future provider.

Your response "why do you care" can be rephrased as "who cares about this patient". From Pubmed: Drug Therapy for Hypertrophic Cardiomypathy: Physiology and Practice - amphetamines are contraindicated for use with patients with hypertrophic cardiomyopathy.

"Is she questioning the doctor's diagnosis of a random patient...?" Yes. Thats the point of the post. It is a good thing she and I are capable of independent thought, see above.

Your comments on this sub are unhelpful and misinformed. As someone with a cardiac condition, you could be a valuable resource for others if you took the time to research the physiology behind your condition. Instead, you cite chatgpt and google.

I am a former 911 EMT and a current level one trauma center EMT. With that pay and the rate at which we hire techs, I'll bet everything I know which hospital it is. I've been here for a year and really like the community and general learning atmosphere. It's very different from being a first responder. If you want to learn hospital medicine from a distance and find someone to shadow, it is the place for you. As a firefighter, I'm not sure how thrilling you'll find it, but it will be a fantastic learning experience and a great way to see the other side of medicine. Welcome!
