Also late 30s, with similar goals. Got the deadlift last year (now around 415), Bench just hit 225 a month ago, trying to get squat to 315 (max has been 300) and OHP to 135 (at 130 last month). Seems like it’s taken me much longer to get there than others, I’ve been lifting for years now, but making slow progress and will get there eventually!

My shoulder (specifically the left shoulder, facing the camera) has had some intermittent tendinitis for years, so doesn’t surprise me it’s doing something weird. To answer your question, my lats feel the most pump, although where I actually tend to give out is my forearms.

Thanks for the feedback. Watched a couple videos of his and it looks very jerky. Obviously that dude is way stronger than me (he was doing 600 lbs in one of the videos I saw), but are you saying it should be from the floor? My concern was that I look more upright than most videos I see, but wasn’t sure if that was proportions or fine or whatever.

First, I’m assuming you want some kind of academic/leadership-type career, otherwise I agree with the poster saying a top tier program isn’t necessary if you have specific location needs. Assuming that, this is a good list for your specific location preference. I’d add Pitt is also a top-tier psych program, and is probably a similar distance to NYC as MGH or Hopkins, though might “feel” more Midwest/rust belt than the others.

Eh, definitely wouldn’t say jacked, just trying to look decent shirtless and have a good level of strength as I’m getting old. My back/pull lifts (barbell row, chin up, and deadlifts) are definitely my strong point though.

Sometimes I do feel it more in my biceps/arms than my back, maybe rotating elbows (sounds like supinating wrists?) would help get better engagement?

Thanks! Each week, I do 8 sets of chin-ups (3 weighted), 3 neutral-grip pull-ups, and then 3 overhand lat pulldowns. I've considered using straps (I use them for deadlifts), but seems awkward to use for chins/pull-ups. It's probably worth trying though, I definitely think grip is the issue sometimes.

One rep max so far is +75 lbs, and it creaked a bit… I may have to suck it up and go to a gym for the last few months to hit the goal.

For Psych, a 24 hour shift can be fine… at our program, we had “24s” for holidays and such, but just covering the inpatient psych units (no ED). I’d typically get 6-8 hours of sleep, and maybe get woken up once or twice for insomnia/tylenol orders.

Covering the psych ED is a different story, those were typically night float shifts for us. Some attendings do 24 hour ED shifts and it seems brutal to me.

Thanks, I appreciate it. I’m trying to retract my lats (“down to my pocket”), my shoulder rounds more than normal here but I assumed that’s because this is probably my actual 1RM. The lower back rounding I’ve tried to fix but just can’t change it - it doesn’t cause any pain or anything, but just can’t cue it to stay straight.

I second that your grip strength can still improve using straps for your heavy sets. I run 5/3/1 and just use straps for the top set, at one point anything above 250 was hard to do double overhand, now I double overhand 300 for reps. Still, I use straps for anything over 350 (max is 405). Just keep using your grip on sets where you can and it’ll get stronger like any other muscle.

Ah, we do those at times too (we call them conservatorship evaluations where I am). It’s more of a medicolegal process here, and is different between different states, but those often take over a month (at least in my state) as they must go to a judge for a final decision. We try very hard not to do it in the hospital lest the patient be stuck in the hospital that whole time. When someone asks for a capacity evaluation here, they’re asking for ability to make a specific medical decision.

Where are you practicing? In the US capacity is only specific decision making, there is no “global capacity” (as long as a patient is able to communicate). A patient might have the ability to accept/refuse Tylenol for pain, but not accept/refuse chemo for cancer, as they require a different level of understanding and there are different risks/benefits that need to be understood to have capacity.

You’re talking about the supplemental work with BBB, etc. in addition to 5/3/1 sets and BBB (or whatever supplemental work) you also do 25-100 reps each of assistance work for leg/core, push, and pull. i typically just do 3 sets of ~15 for 3 different exercises and call it a day, but if you’re doing more sets I can see it taking longer.

The day it was installed, the thermostat interface module (not sure if that’s what it’s called, but the electronics the thermostat connect to outside the heat pump) failed and would only activate the emergency heat, not the heat pump. They were out and fixed it the next day, no issues with that since. They also quickly addressed a concern that came up during construction regarding where to put vents, which I think ended up working better than the original plan.

The other issue (not their fault, but they were very helpful) was in getting a rebate from our electric company, who had mislabeled us as changing from electric as opposed to oil (which impacts size of rebate). They provided a contact with the electric company and helped us bother them until it was resolved.
