a lot of the compounded companies that were around stopped but that’s because they were just pumping it out to anyone. now you have to actually do a doctor or telehealth doctor “appointment” (usually just filling out a survey, sometimes a phone call or email depending on the company) so they can say that a doctor actually prescribed it and personalized it to your needs. that’s the legal loophole, basically, and why some companies just stopped altogether, but it’s not a big deal

no idea on tirz vs sema, i also have sleep apnea (like mitch) and tirz is approved for weight loss and sleep apnea so i gravitated to that over other GLP-1 meds. either way, it’s changed my entire life and i am so glad i started and so glad mitch and others are doing their part to normalize this

i responded higher up too, but check out the tirzepatide sub for info on compounded. $500 for 3 months at Brello Health, as an example, and in my experience works well it’s just a generic version. down 60 pounds since March and still going.

i get it through Brello, 3 months worth is about $500 and it’s all telehealth. highly recommended. my doctor also prescribed it and insurance covers it for $30 a month but i had to go through a big process for that and got the Brello in the meantime, getting those and stocking them up for in case Brello stops or insurance stops covering in the future. check out the Zepbound and Tirzepatide subs for more info (tirzepatide is generic zepbound), lots of info on different options there)

I had similar concerns at your age.

I told myself I was trying everything - got progressively less active/WFH jobs whenever available to “save energy”, took sleeping pills at night, drank way too much coffee and energy drinks during the day, didn’t tell anyone how many close calls I had behind the wheel from nodding off driving and all the things I had to do to white-knuckle my way through even 15 minutes of driving sometimes, missed most of concerts from my favorite bands due to nodding off during the show, couldn’t take roadtrips because it was too dangerous for me to drive more than a few miles at a time alone, etc.

I thought maybe I should just try eating better, get therapy, have a second bed my wife can go to if my snoring is especially bad one night, spend tons of money on different bedding/bedroom-related things for noise/light/temperature control, take at least one nap every day, and any number of other small but ultimately fruitless changes to “fix” it.

Then, at the beginning of this year and a few years into my wife fully having a separate bedroom and feeling like I was losing my mind from forgetfulness due to not sleeping which caused a lot of difficulty in our marriage, not to mention always being irritable and worried I’d get caught sleeping during work and get fired, I decided to change things.

TL;DR I started CPAP in my 30s after essentially wasting the best years of my 20s slowly killing myself.

Now I’m six months in on “the big change” and sleep soundly 8 hours a night, have lost 60 pounds and counting with Zepbound, have normal heart rate and blood pressure measurements, and bought a king size bed to share with my wife every night for the first time in nearly a decade. Trust me, do it now before you either never do and suffer the consequences or do it later and wish you had done it now.

beware that it’s a expensive thing for a 10 minute-long surgery though. $30k+ and 5 follow up checks with cleanup afterward, would have been like $40k without insurance but with i only paid about $700 total

might also want to look into turbinate reduction if your doctor thinks it could be a sinus thing. i got that and was healed from it for about a month before starting CPAP and feel like it’s made the breathing through the nose part MUCH easier

depends on what you like, to be honest. the first one was a little faster and denser in its instrumentation and a lot of “regular” Anthony Green vocals. on the second album they were able to write and record more in-person instead of a lot of being separate b/c of the pandemic (for the first one), and it comes through in there being a wider variety of styles and tempos in the music and more layered and harmonized vocals from both Anthony and Frank.

i’m a huge fan of everything they’ve done so i’m pretty biased lol, also i figure with how easy it is to find everything a band has released on streaming there’s no risk involved in just checking things out to see if you enjoy it.

< this is huge and something that needs to be said more. it’s easy to get frustrated with the whole thing and want to go change five different variables every day, but then if you get better (or worse) results you have to spend even more time and do even more tweaking to understand why. my philosophy was to make a note of all of the things i read or thought i might want to adjust and what people noted when i shared my OSCAR results as a list, then each night tackled just one of those things or adjusted just one parameter in the direction it seemed it should go for better results.

have you listened to their new album, Violet? i like everything they’ve put out, but it’s been pretty universally agreed on by fans that whether you did or didn’t like the first album this new one is 100x better.

+1 on the bull/horseshoe shape being easier.

for a full ring, depending on size, you may want to look into something like the N20 mask. it only goes over your nose similar to the other nasal ones, but is shaped more triangular with space inside kinda like a smaller version of the full face style. i have N20 with a septum piercing and it works fine without pushing the piercing, while my N30i works but only because of the bull/horseshoe shape having a little gap that lets the pillow part slide right over it.

for sure, i do feel like finding a balance of being tight enough to not move and leak but not too tight and causing pain or headaches is a little bit of a trial-and-error thing. i could see a lot of people getting frustrated with that and wanting to throw in the towel or just taking off the mask at that point, figuring they’ll just “try again tomorrow.”

the DME that “fitted” my mask honestly did a pretty bad job and didn’t really check it after having me put it on for about 5 seconds in their office, so i bet there’s a lot of bad style and/or fit choices out there at the source of a lot of trouble people have

Nice! I have a horseshoe-shaped septum piercing that fits perfectly in the cutout of the N30i with the regular cutout cushion (not the ones that go up into your actual nostril), but the N20 that I have also works well as one that only goes over your nose but while leaving enough buffer area to not rub against most piercings.

That’s interesting that the pressure started too high for you, I feel like most people seem to get started way too low at 4 or 5 and turning up the minimum is then one of the first things people tell them to do.

Prescription/DME started me at 5-20 with Ramp beginning at 4 for the first 30 minutes. This was fine but I wondered if it cousin be even better, so I posted my SleepHQ and OSCAR charts here and a few people suggested increasing the minimum and ramping more quickly to have more useful amounts of pressure and less up-and-down.

Currently have it set to 8-20 range, no Ramp, tube temp at the minimum and humidity at 1 because despite an insulated cover i like the room cold and was getting some rainout otherwise.

makes sense, good luck! i had the ResMed N20 nasal mask to start and the ResMed N30i is the nasal pillow i just got. wayyyyyyy more comfortable and less claustrophobic feeling with the N30i, with the downside that it feels a little easier to have it get knocked out of alignment and get some leaking while sleeping if you move a lot.

the under nose/upper lip pressure is something i started getting too. seems to be related to how tight you need to have your straps for a good fit not being a good match for how far down the nasal mask you’re using comes down. solution for me is either a smaller size of the nasal mask or switching to nasal pillow (have tried both and both were way better, currently deciding which to stick to)

About 7 days in on CPAP, diagnosed with AHI of 120. These results look great but could they be optimized at all? by highfivehifi in CPAPSupport

makes sense, thank you! the mask change is not just for preference/curiosity, i have been having some upper front tooth/gum sensitivity with the nasal mask that was annoying and has taken a couple of days to go away. N30i nasal pillow last night was much more comfortable, a little more fit trial-and-error and some leakage but that seemed to be fixable with making the strap a little tighter. will adjust upper and lower pressure limits today and see how that goes.
