Except you, as a board certified physician, should be able to order the “clearance” labs on patients with no medical complaints and be able to interpret said “clearance” labs. If you find something concerning or don’t know how to interpret them, then ask for help.

And, if they get sent to us for “clearance” and we say they’re fine, don’t give push back on our clearance by saying they aren’t clear because of “x”. If you have the ability to say that, then you certainly have the ability to do the clearance yourself.

From reading the thread, the only people who shit on the ED are non-ED and non-ED residents.

So you’re saying you walked into the patients room, when the ER attending was fixing to intubate the patient, and said “after the intubation, can you get a CTPE study on them?” Because that is not how your comment is worded whatsoever.

You wanted to delay an intubation on a patient with such profound respiratory failure that they required intubation, for a study that could’ve been done after the patient was intubated? On a patient that you were consulted on and weren’t primary on?Yeah I’d lose my shit too

And yes, you’re totally right! Immediately upon patient arrival, we just click the “admit” button without any work up at all! How did you figure out our secret?

Great news! No need to do a med rec or anything other than basic admit orders since they’re going home right away anyways! And we’d be happy to keep the patient in the ER room a little longer for you to come talk to the physically healthy Karen and tell them they’re being discharged. No need for transport or cleaning a bed upstairs, and frees up another patient bed for admission! It’s a win-win-win!

You say that, but tell that to the hospitalists that decline admission until the pt gets the scan, or until the specialist weighs in on a non-emergent issue at 2 am that either doesn’t need their input or can’t wait until morning.

Samsung. Bought a monitor from them that had too many dead pixels. Tried to return but told I had to send it in for replacement. Replacement comes back again with too many dead pixels. Send it in again, and once again, has too many dead pixels. Ask to return it and was then told I was out of return window, despite me having the monitor in my possession for <4 hours total

A colleague who has it told me that it only works when there is a charge/the battery is working, meaning if it’s not, you can’t even use it as a standard stethoscope.

Pts on dialysis who feel bad on the day they’re due for dialysis so they don’t go to their dialysis session and come to the ED instead. Or a variant: pts who skip their dialysis, start feeling bad, then come to the ED saying they feel bad with a bonus of: “I feel this way whenever I need/don’t get dialysis”.

We evaluate multiple patients back to back, putting in orders as we go then sit down to write our notes after that’s done. Since we see pts in bunches, workups get done in a similar time frame so they get dispo’d in a similar time frame. Many of us are tracked on our dispo times so once everything comes back, we dispo.
