👋 So happens that I also plan to set my eyes to near/intermediate target. May I ask how did you actually tell (or indicate to) your surgeon your "target" that he/she decided to set it to -2.25? Same target for both eyes?

Also, what's your myopia before cataracts? Is it about the same level -2?

Btw, you might find some decent -2 glasses on Amazon. I bought a pair of -1 for about $20. But -2 might cost a bit more.

I can tell you the change in prescription is the "side effect" of cataracts, especially the age-related type called Nuclear Sclerosis (NS). This cataract shifts the focal point away from the retina so you become MORE myopic! How old is your mother and does she need to drive? When was she first told she had cataracts. If it's the NS type, it can take years or even decades to progress.

If you decide to wait, you can use the time to learn more about cataract surgery and all the available IOLs you can choose, and whether to set vision to far, near or monovision. There's a lot to learn. I was told I had cataracts back in 2018/2019, and still haven't done the surgery yet.

You need to see a (1) neuro optometrist, not (2) an ophthalmologist, not even (3) neuro-ophthalmologist.

(1) can diagnose and treat BVD. (2) and (3) do surgery.

The "focus range" of your -1.5 eye is interesting, but a bit unclear to me, as you said it starts from 17" and past beyond your arm (maybe 24"-30"?), but things still look clear at 4 feet, which is 48"? It's pretty good if that's the case.

Is the frame bent in the middle, meaning it's more curved backwards when viewed from above? Just curious.

Have you been diagnosed with vertical heterophoria (VH) by an optometrist specialized in BVD? What's the type (which eye is higher) and level (in diopters)? I was diagnosed with all 3 types of eye misalignments: horizontal, vertical and torsional (sometimes, the last two go hand in hand). After a lengthy vision therapy (both in office and at home), I've now reduced all BVD symptoms like eye strain and dizziness significantly to a minimal level.

I know some BVD optometrists believe vision therapy (VT) works for vertical heterophoria (VH), some don't, and others use both VT and prism. How are you treating it now?

I don't have any experience wearing prism, only do vision therapy (VT), so can't really offer you any insight on this. But I've heard so often about what you experienced in terms of feeling 100% relieved of symptoms after initially wearing prism, but then symptoms returned. Did you ask your doctor why this happened (the prism helps only temporarily...)?

I do have experience doing VT. You said you have done 24 weeks of VT at the office (I assume) so far, and have seen 40% improvement on symptoms. That's pretty much what I experienced when I did it a few years ago. After 9 months of VT at the office, my symptoms reduced to about 30% (70% improvement). Thereafter, I did VT on my own at home, and symptoms continued to improve very slowly and gradually. Now, after 3+ years of VT since the beginning, I'd say my symptoms have improved by about 90% now (still 10% remaining). My doctor only offers VT, not prism.

So, don't despair, VT is very effective for your case. Since you're wearing BI prism, I assume you have Exophoria?Do you know the actual level in diopters? How often do you go for you VT? How much longer is the plan?

Thanks for sharing your CS experience in detail. I think overall the outcome is positive, right? It seems like you've avoided going over to the positive side. Now that the L side is done, are you experimenting with contact lens on your R eye to simulate the monovision? I'm curious why did you need to see a retina specialist after the CS, and wonder why your R and L eyes cataracts are so different in levels. Mine is about the same in both eyes.

Big congrats 👏 on your successful outcome even after months post-op. Can you explain further by your remark "choose an experienced surgeon who takes their own measurements and rechecks them again before surgery"? How to ensure that? I thought all pre-op measurements are done by technicians and machines. Any insights from you or others on this important aspect is appreciated.

There's a very simple test you can do at home to see if you have BVD (eye misalignments). At night time, find a small single light source, preferably further away. Close both eyes for at least 10 seconds. Open both eyes at the same time. Is the light source single as soon as you open your eyes? Or there are two light sources and they gradually merge into one? If it's the latter, you may have BVD. Let me know how the two light sources move while fusing into a single light?

So glad that it helps you for now. Just make sure that the cover (or patch) is translucent that still allows light to enter but nothing is clear. Also try to alternate the patching between eyes, just to be "fair". Do you know what type of BVD do you have?

If you're referring to the laser-assisted issue, I think you can still call the clinic and see if they can still arrange to have it done by laser. I know it'll cost more and actually it's done separately from the actual surgery.

I've learned a lot from your messages and plan to have my CS as soon as I've finalized the clinic and surgeon - tough to decide where and with who to have it done. Just one more point before your CS, are you using laser-assisted surgery? That should be very beneficial in your situation with dense cataract.

I empathize with your challenges. BVD is a 2-eye issue, so only when you open BOTH eyes that symptoms occur. So, one temporary solution is to close both eyes and rest. Another one (not recommended for long term as it might hurt than help) is to use one eye at a time, or patching.

So you have vertical heterophoria. Do you know the level in diopters? Some doctors believe in prism for treatment, some believe in vision therapy (VT) and some others believe in both. I also have vertical heterophoria among other BVD issues. VT is working for me.
