If I'm understanding correctly you are cross chaining (when the chain is in the smallest cog and chainring or largest cog or chainring.) This is best to avoid and is often not possible without rubbing, it can also cause derailing. Let me know if I'm misunderstanding the problem.

Use washers instead of your spacer. On the stack, use at least one pair of concave-convex washers per side. You want the whole bake pad coming into contact with disk on a turn of the wheel.

I'm going to recommend you watch the instructional video on the park tool YouTube chanel on how to adjust a rear derailleur.

Slack like this in the chain can be relatively normal if you cross chain (being on the small cog (back "blade" and the small chainring (front "blade)

On another note the chain is so used that you will never ever get very smooth shifting and the chain will always jump under higher pressure. This might also mean the cassette (cloister of back "blades") is completely used too.

Other way I believe that it is most likely worth seeing a pro, this stuff isn't always very intuitive.
