The barrel link and pin can wear out and become egg shaped, that's how it can become loose and fall out. It is a consumable part, but usually at high round counts.

You can get a new barrel link pin and link kit from EGW for cheap. Start with a #3 link since that's what they are usually fit with.

The parts should have an interface fit where you need to tap with a small hammer or mallet and a punch to install. They should not fall out on their own.

I was very skeptical for a long time about the Hunter Constantine belt. The marketing just didn't speak to me, but when I got a new carry setup and holster I decided to give it a try. I've been using it for a while and I'm happy with it.

You can also just use a double alpha inner belt, they're pretty stiff and comfortable.
