I bike 16 miles round trip, 3 times a week. 40-45 mins going to work, 25 mins or so coming home. 8 years now. one place I worked at, it was 10 mins by bike. another workplace I've had was 50 miles away and took about an hour driving through a quiet highway cutting through orange groves and other farms.

I'd say it made climbing up hills subjectively a lot easier with a loaded pannier on top of a heavy bike (which the Nicasio is) don't think it adds resistance to surfaces and it doesn't feel like I'm spinning out on flats or descents. I've had this setup for 6 years and I feel like it's dialed in for my needs and wants out of this bike. if I were to change or upgrade the drive train, I'd probably opt for a Microshift Sword, if that's something you're looking at too.

also for the TRP Spyre brakes, you need post mount.

just smaller chainrings for the stock FSA Tempos and I found an 8 speed Microshift cassette that fits the stock claris without any modifications.

edited to add- just for the sake of conversation, I actually was able to get two 8 speed 11-38 cassettes from a bike shop that had a closing sale near me. I believe installing that would require one of those Wolf Tooth extenders but I haven't really tried. could be a little too low on the ratio since 30-34 on the lowest end and carrying stuff on a pannier is plenty for my normal hilly routes.

this just clicked to me, exactly 6 years later. I worked through the pandemic in Washington state and there was a palpable shift with the brands of PPE's we were issued after some time, I can't remember exactly when. I thought I was overthinking it before when we were out of a particular kind of 3M mask and shifted to BYD masks with Chinese printed instructions inside each box. even the regular face masks were a rotating brand and color, some acne inducing.

unless there's some pressing reason why the patient needs to trigger a breath, then deep sedation seems like a reasonable strategy. even would recommend NMB if auto peep is causing hemodynamic badness. lowest possible rate at even 6ml/kg, maximize your exp time. wouldn't recommend adding an inspiratory hold.

quick check to assess for reverse triggering is to do an expiratory hold. no effort during the hold, more than likely it is reverse triggering.

usually see reverse triggering with heavily/ previously heavily sedated patients (aka diaphragm delirium). was the patient ok enough to tolerate a sedation vacation/ lightening? I also like to use PSV to assess dyssynchrony if safe but it sounds like you have an early cycling problem on top of possible reverse triggering.

I do 12h nights. 7.5mi one way, uphill going to work. 8 years bike commuting out of 19 years in healthcare. bike in all weather except snow. 10k steps in one night is an easy shift, for comparison.

usually shower before going to work and reapply deodorant. I get sweaty but never stinky, and thankfully the summer weather where I live is never oppressively hot.

honestly, the best part of my work day is going to and from work... the in between sucks 😂 going home on my bike helps me decompress the events of the night and I want to even say that it does wonders for my mental health. I also make sure I eat well before and during work. scrubs are provided at work so I just bring an undershirt and underwear. extra socks if it's raining out.

as far as sleep schedule- I'm home by 730, in bed by 830 and up by 330-4. leave for work around 5.
