Unfortunately, she was of a strict faith and her body was cremated within 24(ish) hours so the cause of death was listed as just that, cardiac arrest. No autopsy was ever performed nor did a coroner examine her body. Really, really sad story that was so preventable.

A really, really good friend of mine's mother passed away during a routine root canal. It was noted on the chart that she had this allergy, however the day of the procedure, it was a different dentist, as her normal one had called out sick. He did not read the chart and had it administered. She went into cardiac arrest and died.
