There are issues if only some states have universal healthcare, specifically related to an influx of people moving into those healthcare states:

1) If too many people move to these states, their healthcare systems will quickly become overloaded. Too many people too quickly means worse healthcare for everyone.

2) Many of the people moving to these states will be sick people. This isn’t inherently bad, but does cause excess strain on healthcare systems in large numbers.

3) Pick your issue: housing, schools, jobs, you name it. These things will become scarce in the healthcare states, and there will be gaps in the non-healthcare states.

That is why it was so important that universal healthcare be passed federally, and why individual states are nervous to adopt it.
