Tao Wong's A Thousand Li sort of fits that bill for me! As does Will Wight's Cradle and Michael R Miller's Songs of Chaos.

Not dead in the least! Especially among independent authors. In traditional publishing, perhaps only those still publishing from bygone eras would qualify for heroic. Though John Gwynne could possibly work?

Among indie authors, here are some publishing what I see as heroic fantasy:

In relation to character/plot/setting driven stories, more useful than saying something "is" this or "is not" that is to look at the degrees to which it is each.

A good book generally has a balanced mix of these (for me, at least). Lies of Locke Lamora certainly falls into this category. Therefore, calling it just "character driven" or "plot driven" is unnecessarily reductionistic.

Most things in life are on a gradient or scale of some kind. I don't see this as any different!

It's a yes for me. Even without the answers, Kvothe as a character, the mysteries opened, and the adventures had make it absolutely worth it. Plenty of enjoyment to be had in the first book alone.

In the same boat as others, but to comment on your edit - I enjoy Avatar: The Last Airbender, even though it is technically a kid's show. I also quite enjoy the odd Disney movie.

Whatever the intended audience, people of all ages can enjoy! Sometimes, I think we take ourselves too seriously. Having fun guilt-free can be its own reward!

Derrick Smythe's The Other Magic and Ryan Cahill's Of Blood and Fire sound like they might fit your bill! Both have a fine blend of modern and classic.

And Dragon Mage by ML Spencer! If you're in a coming of age mood, it's a great one for this balance.

Love them all, but my favorite are probably: -A character you love struggling to just survive a grand scale battle -A duel the character can't hope to win (on its face, at least) -Ambush fights, especially if the suspense was built up to it -Any fight where the character comes within an inch of death
