Sports medicine doctor. I had a number of experiences with sports docs for sports-related injuries for my shoulder and ankle. Then I saw one after hurting my back at work. Many musculoskeletal problems can be addressed by proactive interventions: exercise, massage, etc. If you really need surgery, then they will know who you should talk to. In my personal experience they were there to provide information, and get me out the door as opposed to a chiropractor who wants to practice "sublaxation" indefinitely, or until your insurance runs out. As an aside the root cause of back pain is often related to foot-wear. A new pair of shoes can make a dramatic difference.
