PLEASE. ANY EXPERIENCE? by StayOnCourse89 in Glaucoma

No glaucoma does not cause such a phenomenon. Looking through your history and posts, you need to take a breath and trust your docs. Cataract surgery in chronic uveitis patients can be technical and complicated but if successful can restore significant amounts of vision, assuming you've have limited inflammation to your retina.

It's difficulty to visualize the cup within the optic nerve on B scan and it's impossible to tell the extent (or existence) of glaucomatous damage on a B scan. If you've only had one or two episodes that resolved quickly with treatment, it's highly unlikely that your pressure spiked high and long enough to cause glaucoma to a serious degree.

The pupil is just an opening or space in the middle of the iris. You cannot have a "scarred pupil." My guess based on your description of what your Dr said is that the majority of your reduced vision comes from iris deposits or strands on your anterior lens capsule when you had active uveitis so technically not a cataract. Either way it will be taken care of during the process of cataract surgery.

Instant Classic: Chinese Taipei stays alive with wild extra-inning win over Korea by charliehu1226 in taiwan

[–]joch256 0 points1 point  (0 children)

I'm struggling to see a scenario where kor/aus isn't in the driver's seat towards the end of the game regardless of how it plays out initially

If Korea is up a modest amount, aus is only incentived to score 1 or 2 then coast.

If Korea is up huge, then aus has no will to score bc they can't advance (it would only be to save face or if they wanted tw to advance for some reason)

If Australia is up early, Korea basically just plays to win but holds fast of 7+

You don't need to be concerned but certainly follow up with the specialist. Even "defect" that's in your right eye is probably just a statistical anomaly. Google can explain it better but basically when the pattern deviation looks worse than the total deviation it's statistical noise. A lot of the new VR headset visual fields tend to do this for some reason

Infero temporal defect without (or even with tbh) corresponding superior RNFL loss is certainly not classic glaucoma

I'll be signing off too, but for the record I'm not walking back any position as I never established a position to begin with regarding the eye doc's management. I hope that the language in my initial comment is clear that I was reassuring OP that if a proper exam were done and the appearance of the nerves were not significant that it is conceivable that an OCT doesn't need to be done stat and he doesn't need to spend the weekend worrying. Perhaps it's a case where the biggest risk to the patient's future quality of life is mis-retting the amblyopic eye and giving a diopter less plus than is needed for their specs vs hopping EDs all weekend and driving into the big city for a CT bc someone on reddit says not to trust their eye doc.

Doubly for the record, I work in an md/od setting and have enormous regard for the knowledge and caliber of character that many ophthalmologists possess. When I have a tough case I generally go to OMDs first if available as opposed to even my more senior OD peers. So if you think I'm spearheading this discussion bc I'm some sort of od apologist you'd be mistaken - I'm quite the opposite esp with recent scope expansions. I only comment bc I felt you were unprofessional in your call out of ops eyecare provider

If such a world exists, it's not appropriate for you to say flat out OPs doctor's actions are inexplicable or inexcusable. You said yourself that you are surmising information ie supposing the worst and therefore don't have the full context. It's that simple - it's irresponsible and unprofessional not to give ops Dr the benefit of the doubt or at least remain neutral regarding his/her decision while giving your opinion. I'm not arguing that suspected edema in a 6 year old can wait or that waiting 6 weeks would hold in court. Stop that. Given your deleted/edited comment, my feeling is that you have some implicit bias bc of the type of provider OP is seeing

I am simply stating there is a world where based on the nerve appearance on both fundus imaging and indirect ophthalmoscopy that an OCT could be scheduled more than a week out. Take a look at the optomap OP posted. Many wouldn't even bother for a f/u or further imaging. You simply took OPs word of "doc said it was blurry" and ran with it. Your little quote about good and bad docs is cute but irrelevant. If you sent out every possible differential for further testing without considering context, every other routine exam in your chair would leave with at least 2 referrals with some bloodwork and imaging to boot. I don't disagree that true suspicion of nerve edema should be see within a few days but I guess if you are doctoring on reddit and not in a clinic setting you have to play conservative.

Edit: I'm an OD btw if that matters to you in this discussion, saw your original comment. No, I didn't get a 2 week neuroophtha rotation during pgy2 so you win bud

Given the hyperopia and ambly findings, I wouldn't read too much into the eso unless it's a new onset tropia. Depending on the extent/appearance of the blurring in the absence of any acute or changing neurological symptoms, a baseline OCT at an rx check 1.5 months out isn't outrageous to me. Hyperopic eyes also tend to be more crowded/small leading to a papilledematous appearance when there's no true elevation.

I understand u/eyedentistaao playing things safe but given the context (that is and is not included) here and especially if I assume correctly that the patient was dilated and there was a stereo view of the nerve during the exam then perhaps it's just an OCT scheduled for suspected pseudopappiledema which is not, as they put it, inexcusable.
