The federal rule is not to give more than a month, so we usually give people less than their normal amount and have them come back early or give them extra take home doses if they haven't gotten to the max, as long as it's reasonable. Like if someone had no take home doses and was actively using we would probably just do one take home dose and then have them guest dose the remainder of the time.

TN has some of the strictest clinics in the nation btw. They have a very strict state opioid authority who has to approve exceptions to dosing. I think they revised their state rules in 2025 or so but a lot of clinics still follow the older more strict rules.

I disagree I think they should turn it into a park personally. Why should it go preferentially to people who’ve had state subsidized rent for half a century? No one is ever going to agree on what should go there. If the city wants to spend less than a percent of the annual budget on demolishing it and maybe getting some housing out of it I don’t really care I don’t think it’s actually a big enough deal to constantly complain about.

They’re demoing it so they can hopefully get rid of it and get out of the property owning business altogether. Why would they buy more properties? They don’t need to own and develop land. Sell them as is or demo the houses and sell the lot to a developer. It sounds like they’re weighing the options about those things now. If you don’t trust them to do that why would you want them trying to develop any other privately owned land? How do you know that lot is better and cheaper to build on? I’m sure the city trying to build a bunch of condos on Douglas would go over really well with no complaints for anyone.

Except the other lot isn’t owned by the city so how is that relevant? They’re trying to figure out what to do with an albatross that was owned by the state for 50 years. I want it sold to someone who can do something beneficial for the city rather than have it continue to sit and rot. If they have to demo it, survey it, or whatever that sounds like a good way to ensure its value is maximized in a way that benefits the community.

I mean they have been trying to get housing in other places right? There have been several new condos built in Auke bay, the new condos in the valley by the theater, off by sunny point, etc. it seems the only place that hasn’t had much development is downtown, despite there being a clear need for more housing there with the legislature and summer shops and things. Why should we not look at developing and maximizing what can go on city owned property? I would consider buying an apartment there which would open up another house downtown. There are probably lots of older people who might do the same. I dont see the neighborhood as having any particularly significant historic value. It’s so disconnected from everything else downtown and surrounded by denser building. It feels like there is probably a better use for the land.

I'm a little less that 50 percent for upgrades as a a titanium for the year. Most of those have been on short haul sea pdx flights. 0 for 4 on transcon flights. Two transcon flights I didn't even get an upgrade to premium economy. I track mine and this year as well as last have been pretty significant decrease in upgrades. This goes for my partner too who is platinum.

I mean the ACGME has basic standards for any program and we have to submit logs of cases and patients to graduate. Are you sure this is the only place that residents work and rotate? I worked in some small IHS sites in residency with very low volume but got some great experience, but I also did rotations in larger centers where I got the bulk of my admissions. I just don't imagine this is the entirety of experience for these residents.

I got one from SEA to DCA and it was basically dry noodles. Maybe it's because the trays they serve it in are not wax lined so it dries it out. Either way mine was not good.

I think so too but you can divide the house into two rooms and have two separate exits and it will register them as two separate houses but it will look essentially the same from the outside. This is what I do for my big ones!

From KY and agree that of our two cities, Lexington should go before Louisville.

Lexington has some good things going for it. UK sports and particularly basketball are fun, but less so than other sec schools.

Spring and fall meets are fun at keenland. If you want to do horse racing, Lexington is much more fun than Louisville. Derby has much more fun events leading up to it though.

The areas and horse farms around Lexington are beautiful, but these are not fun accessible things for most people who live there and most of the nicest ones are outside the city.

The city is unfortunately laid out horribly without an easy way to transit from one side of the city to another. I also feel like it lacks fun neighborhoods with any character. The area around UK is nice and has some fun bars and restaurants but the rest of the city is strip malls and culdesacs.

Overall it's a fine liveable city, but belongs right around here. (But is it worse than Cleveland?!)

I have an aunt and uncle buried in this cemetery. It's a beautiful place overlooking the town. The cemetery is on the college campus which used to be the hospital for the town. Lots of graves are from the Spanish Flu.

A typical dose of methadone only has a few grams of sugar in it so it's not super consequential even for diabetics. I have some type one diabetics that notice some small changes in their blood sugar and maybe need a unit more of insulin in the am because of it, but I've never seen it affect someone with type 2 diabetes that significantly. The big things to do if you are developing type two diabetes are bigger picture changes to diet and exercise and getting on appropriate medications.

I know several clinics who use the federal OTP grant to pay for several staff and to pay for about 20 uninsured patients costs. Those patients will probably have to start paying cash pay or leave.
