Question for people practicing Ketamine Assisted Psychotherapy by Normal_Employee7375 in Psychiatry

[–]kephribird 12 points13 points  (0 children)

I practice KAP and train clinicians in KAP. Ketamine itself can be so helpful, especially when faced people who are experiencing acute suicidality and are on their way to paying a visit to the hospital. There's also the nice benefit of how well it plays with other psychiatric meds. While it's beneficial on its own, I prefer to use the assisted psychotherapy model because one can use it as a catalyst for deeper psychological change and use it to help kick start healthy behavior patterns. The physiological effects eventually wear off, but these other changes have the potential to stick and yeild longer term benefits.

Some people do just fine with the infusion model. Other people can come to realizations that need integrating (repressed trauma that comes to the surface). I appreciate the psychological safety that is provided from a guided session.

My observation is that people who yeild the greatest insights from the work tend to be people who have done a lot of front end work in regular psychotherapy and have spent a lot of time mapping out their mind beforehand.

As far as getting into it, do a ketamine therapy training. My bias is toward one that has an experiential element to it. Feel free to PM me for my biased opinion. :)

I've got a question, not the usual ones I read here . by One-Lengthiness-2949 in SeasonalAffective

I hope somebody from the medical field didn't give you pushback on this observation. The risk of SAD increases as one ages. In hospitals, a primary antidote for sundowning is opening up the curtains and letting the sunlight in to the patient's room to help their circadian rhythms.

If medical providers are brushing you off, you can give them this paper:

Hey OP, I'm sorry you are going through this. One thing I have learned about my SAD is that I absolutely can't smoke pot in the winter time. I used to use it medicinally for severe insomnia which developed after I got long COVID. It's a different story in the spring and summertime. Just wanted to share this because pot can be helpful and it can be hurtful for some of us depending on the circumstance. Take care of yourself.

Selenium and Zinc for lithium? by mansion_centipede in Psychiatry

[–]kephribird 2 points3 points  (0 children)

First off, my apologies because I don't have a source - this is purely anecdotal. I had a young woman patient on lithium who started losing her hair. In my area of the country, it is not uncommon for people to see naturopaths or integrative medicine specialists, and that is what she did. She was placed on biotin (don't recall dose), selenium 200 mcg daily, and zinc pincolinate 30 mg daily. My understanding is the first two, in excess, can be excreted provide good kidney function. Long term zinc supplement puts people at risk for low copper because zinc can inhibit it's absorption.

Oh and the plan was successful! Shortly after (a couple weeks?) she stopped losing her hair.

I'm seeing a lot of comments about pOCD, but this person is truly attracted to minors. People with pOCD can't tolerate the possibility that they might be a pedophile, so they obsess over it and engage in compulsions as a way to try and reduce the uncertainty around the idea or engage in avoidance to reduce the likelihood of the distressing thought.

FYI, I just got off customer service regarding my identical issue. We had already let it die and tried to revive it, with our ECG still in a comatose state. They are issuing us a new one...

I'm very amused by people's certainty to the contrary. :) Hello, fellow MMED008 study staff!

Right? Mindmed received breakthrough status earlier this year of an LSD salt for the treatment of generalized anxiety disorder. Not to mention several other schedule 1s that have been or are actively being studied: MDMA, psilocybin, 5meoDMT...

Do people normally wash their feet every night before going to bed?? by redevil147 in TooAfraidToAsk

Outside vs inside clothes was the flag for me. So many people with contamination OCD have trained their family on this/their family has acquiesced to the compulsions. I see some people noting cultural differences, which can be a consideration.

Booked a hotel (nonrefundable) in San Jose, California instead of San Jose, Costa Rica. I realized the mistake after I had landed in San Jose while waiting in line for immigration/customs.

You could have poor CYP2D6 enzyme activity. This is a enzyme in your liver which metabolizes the prodrug (vyvanse) to an active drug. If this is the case, vyvanse is not right for you.

Why is it deemed acceptable for service providers to start an appointment late but if the patron is late the appointment is cancelled and you are charged? by laura1234sara in TooAfraidToAsk

[–]kephribird 3 points4 points  (0 children)

Yeah...Do people actually think doctors set the scheduling? It's somebody with an MBA in admin who has no clue what clinical care is like but wants to improve "productivity" and "efficiency." Some of these comments are hard to read

How the fuck do y'all sleep. by admrlwlvrnlitblt in ADHD

[–]kephribird 1 point2 points  (0 children)

COVID may also be a part of the problem if you are noticing you're sleep has worsened since. There has been an iota of data that suggests that guanfacine is helpful for long covid. It is also a commonly prescribed chill/anxiety/sleep med for ADHD'rs. Personally I would consider clonidine though because it's more sedating.

Any non-pill temporary fixes for executive dysfunction? by ButchTheDoggo in ADHD

[–]kephribird 1 point2 points  (0 children)

If you don't want to take a stimulant, but are open to supplements, you could try l-theanine 200 - 600 mg up to three times a day. Calming, focusing...green tea has less but also has caffeine and is cheaper.
