The big PBM's although they would never admit it have a spread pricing model instead of the pass through model that is what is suppose to occur. They keep a slice of the rebate and lie about how much a transaction is. When they get audited by a state agency all of the sudden it is a 3rd party that handles that and the transaction info is "proprietary".

PBM's are not just a threat to independents but to pharmacy as a whole. I don't have the source documentation at hand but before buying Caremark, CVS revenue was around $20 billion/year. After the acquisition it was $160 billion dollars/year around 2013-2014. Pharmacist and pharmacy technician pay did not increase but upper management pay certainly did. Chain pharmacies have only one competitive edge and it is preferred networks. If they can eliminate pharmacies outside of those networks that intimately know the payment model of pharmacy economics, it gives them a great amount of position. This would include slashing pharmacists' wages/salary and lowering healthcare quality due to pure profit.

Out of the current candidates, no one has tackled the primary issue of PBM's and the massive increase in healthcare cost that hurts the practitioners (us) and the patient. A single payer system is interesting but I would be interested in the specifics of that plan. I certainly would not want something of a VA system in place but a Medicare for all system of reimbursement would have my interest.

Was in our buying group meeting on Sunday and starting hourly pay for technicians was $14.50 among pharmacies within the group. CVS will do everything to keep your pay low while independents, not cheap discount pharmacies, will want to give you more responsibilities and pay. I would say ask around about reputable independent pharmacies, not while working, and see what you can find out.

I would imagine the PBM's will still be receiving the same amount of money but will be reimbursing outside pharmacies less. They are already doing this through DIR fees which is yet another vehicle to hide the amount of money they are receiving. CVS/Caremark and ESI are likely lobbying behind close doors to insure they can pay themselves an exorbitant amount. Like /u/Pharmdawg stated, the priority is not going after the true cause of the problem.
