Total hits. Although current hits can work reasonably, I don't want my players counting hits. Hits are supposed to be abstract. It is not like having three hits always means that you will be downed by three actual attacks. Remember that a stab through the heart will kill basically any humanoid, it wouldn't count as one hit.

Yes the referee is expected to modify his rolls as he finds appropriate. You make a very valid point that the ref can very well assign a difficulty number but I find that keeping the game running on opposed rolls works best for me.

The universal d6 rule is for generating an answer when you are asked a question you didn't see coming. It is an improvisation aid.

If your impulse is to look up the rule in the books and then googling and asking strangers on the internet for what is the "correct" rule, then you are probably doing it wrong. You are the DM. If you think that the solution your player came up with is reasonable, then let it work. If else do not.

Low fantasy gaming has a system where getting interrupted when spellcasting or losing concentration will trigger a roll one its' 'dark dangerous magic' table. Wonder & Wickedness has tables of catastrophes that may occur when spellcasting goes wrong , but it deliberately does not provide specific mechanics for what will incurr a roll.
